Struck by... (Not Elsewhere Classified)

EE(1) and EE(2) were drilling holes in rubber to attach to the scalping screen. EE(2) was drilling while injured EE(1) was holding the rubber. Another EE(3) was just outside of scalping screen. EE(3) could have been used by EE(2) and EE(1) to communicate before drilling. This resulted in EE(1) getting their right palm injured by the drill. EE(1) got three heavy duty stitches.

Over-exertion in lifting objects

Lifting a 4d battery out of a komatsu 480 loader. EE hurt EE's lower back during this. Drove self to the dr. @ 11 am that morning and got x-ray. Had an MRI scheduled that friday. x -ray showed herniated L5 disk.

Struck against a moving object

Operator was in a 490 Komatsu excavator loading a 405 Komatsu haul truck for production. As operator was loading the bucket slipped off of a rock. When the bucket slipped, the machine jarred, resulting in a muscle pull. Operator was able to drive self to the doctor. X-Ray's were performed to verify only a muscle strain. Operator was off for 2 days and returned to work.

Struck by falling object

Rock Fell from overhead screen deck and struck miner. EE did not lose consciousness. Had a laceration to EE's head. Was taken to Hospital to be safe. EE did not need stitches and was checked to make sure EE did not have a concussion.

The delivery driver was exiting truck when ee slipped and fell to the ground, fracturing left hip

Setting caterpillar water pump, the fill pipe swung and struck employee in the shoulder/neck area.

Was changing hammers on Stedman impact crusher. Ratchet slipped and finger was caught between 3/4"" drive ratchet and outside edge of crusher. Resulting in a loss of distal phalange index finger.

Slipped while climbing up to operate a piece of mobile equipment. Initially thought just sprained ankle but it ended up being a broken ankle.

Strain of the calf muscle. We were setting a pump due to the extreme rain. He pulled his calf muscle while placing pipe. The slope (which was not extreme) may have helped cause the muscle strain.

Employee was operating the Track hoe in the pit. He backed the machine over a 2' to 3' diameter rock. The resulting jar caused him to tear abdominal muscles. Unsure of the tear's severity until MRI is performed.

EE STARTED HAVING CHEST PAINS. HE NOTIFIED HIS FOREMAN HE DID NOT FEEL WELL & DROVE HIMSELF TO HOSPITAL. THEY CHECKED HIS HEART & SAID IT WAS UNDER STRESS, BUT HE HAD NOT HAD HEART ATTACK. THEY PERFORMED ANOTHER EKG LATER & EE STARTED TO HAVE A HEART ATTACK. FLOWN TO 2ND HOSPITAL. 7 BLOCKAGES FOUND. BYPASS SURGERY ON 6/29/05 (DELAYED DUE TO PNEUMONIA). DOING OK AS OF 7/1.

REACHED ACROSS CONVEYER TO REMOVE ROCK. HURT BACK DUE TO STRETCH. ALREADY HAD A PREVIOUS BACK PROBLEM FROM NON-WORK RELATED INJURY. REMOVING THE ROCK REAGGRAVATED THE INJURY.
