Struck by falling object

Employee was moving equipment cylinder using a forklift and harness. The harness broke and fell on employee's fingers on both hands. Employee was taken to the hospital. Broken bone in right ring finger & laceration to middle left finger. He was treated and released. Returned to work on 11/21/08.

Over-exertion (Not Elsewhere Classified)

Employee was climbing into the bed of the maintenance truck to clean it out. He states that he felt a ""pull"" in the left side of his stomach and it started hurting. He kept on working for awhile but it didn't stop hurting. He reported the injury and left work at 1:30 pm and went to the doctor. (This 7000-1 is being submitted late due to an oversight.)

Employee was cutting a rubber hose with a pocket knife, when the knife slipped, cutting his thumb.

Employee was working inside sand screw when he slipped and fell, hitting his rib area. He has two broken ribs.

EE driving 773 B cat haul truck with window down, and felt something go into his left eye. He left work went to dr. Flushed out foreign body prescribed eye drops.

While getting on a 980g front end loader he slipped and his left knee hit the door latch causing contusion. 5/2/06 EE received medical attention. 5/8/06 he missed first day of work.

Struck by falling object

Employee was taking drive shaft out of a sand loader to repair it. He reached in to retrieve a seal when the drive shaft fell on his left hand. His left hand was cut.
