Employee was moving equipment cylinder using a forklift and harness. The harness broke and fell on employee's fingers on both hands. Employee was taken to the hospital. Broken bone in right ring finger & laceration to middle left finger. He was treated and released. Returned to work on 11/21/08.

Employee was climbing into the bed of the maintenance truck to clean it out. He states that he felt a "pull" in the left side of his stomach and it started hurting. He kept on working for awhile but it didn't stop hurting. He reported the injury and left work at 1:30 pm and went to the doctor. (This 7000-1 is being submitted late due to an oversight.)

EMPLOYEE WAS CHANGING OUT THE CUTTING BLADES ON A RUBBER TIRE LOADING BUCKET WHEN HE STRUCK THE BLADE WITH A SLEDGE HAMMER TO KNOCK IT LOOSE,A PIECE OF METAL FROM THE HAMMER BROKE LOOSE AND ST RUCK HIM IN THE LEFT LOWER JAW CAUSING A PUNCTURE WOUND.
