IE and another EE were moving the drill when the drill weight shifted and fell over on IE's finger pinning it against marble surface

While working on the T-Block taking marble off of block a piece of marble laid over on thumb an mashed thumb. Then while standing by header saw passed out an then fell an hit head on the floor.

While the operator of the P.C. 400 was moving it he heard loud noise, then he turned to see and a piece of metal bounced off the wall and hit him.

While sliding marble on top of rock, marble fell over and mashed thumb on left hand.

Bent to pick up a piece of marble 51"" and felt a pull of right side abdomen causing pain on right side.

Struck against stationary object

Taking marble off hoist. Pinched finger with marble and hoist. Cut right pinky finger and had to have three stitches.

2007 Annual audiogram testing showed a shift in hearing on L-29 & R-19. Advised by OCCU-tester to send to hearing clinic to be tested. Finally 1/22/08 was confirmed by doctor loss was job related and required to report to MSHA.

While working on polisher, while bending down to put marble in crate, hurt his back from turning and twisting up and down. Was not notified that injury was work related until Dr. faxed report to office 4-26-07.

While welding on track of PC 400, then chipping weld, a piece of hot weld flew up under his safety glasses and into his eye.

While grinding a piece of metal a little piece came under safety glasses into his eye.

Fall onto or against objects

Employee was walking to the drill & tripped over a hose causing him to fall forward. EE tried to catch himself on the wooden bench used for seating. The bench rolled over & hit the EE in the rib area on his right side. This caused a fracture to one of his ribs.

Employee had just finished drilling hole and was walking along walkway at top of quarry. He slipped and fell into a void in the rock. Employee fell head first, hitting a rock, knocking him unconsious for 10-15 min. He has serious cut between eyes, possible fracture in nose, swollen right eye and bash in head.

Struck by powered moving object

Ee was exiting doorway to t-buck while forklift was making it's way or ramp to doorway. The honking signal was not given in time and the ee was hit by the forks in both knees. one knee (R) is just bruised, (L) badly bruise. xrays show no briken bones althought mri is pending to determine damage to cartilage.

Fall from machine

Employee complained of dizzyness as he tried to get off of excavator and lost his balance. Employee fell 4 to 5 feet to the ground cutting openhisarm and hitting his head and shoulder. Employee was planning on fueling up excavator at time of accident.

Employee was holding a piece of pipe stable on the ground with his foot while a co-worker was hitting the pipe with a sledge hammer. The pipe came loose from under employee's foot & bounced up hitting the top of his foot. Swelling did occur & employee complained of stabbing pain.

Employee was using jackhammer when vibrations caused loose cracked rock to fall. He tried to get out of the way, but was too late. One large rock fell and hit him on the elbow, causing swelling and a small cut. EE is awaiting his surgery date for a pinched nerve.

Absorption of radiations, caustics, toxic and noxious substances

Employee complained of possible insect bite on right shoulder. After two weeks of swelling, she reported incident to office.

Fall to the walkway or working surface

Doctor prescribed medication, which caused victim to pass out. Victim hit fire extinguisher when falling, breaking one tooth and bruising his jaw. Hit his head on the floor, needing one staple.

Fall onto or against objects

Employee was in quarry/pit standing on a marble block on edge of 5'-6' ditch. The block gave way and employee fell into ditch on another piece of marble. Complained of pain in left foot, left hip, right leg, right wrist and back. Employee was not able to move. Ambulance took to hospital.

Over-exertion in lifting objects

Carpenters were told to carry crates from woodshop to plant. When lifting one crate under 25 lbs, employee felt a snap in his shoulder area.

HE WAS SETTING UP A DRILL WHEN ANOTHER EMPLOYEE TOOK A WRENCH AND HIT IT AGAINST THE GROUND CAUSING A ROCK TO FLY UP AND HIT HIM IN THE LEFT EYE.
