2007 Annual audiogram testing showed a shift in hearing on L-29 & R-19. Advised by OCCU-tester to send to hearing clinic to be tested. Finally 1/22/08 was confirmed by doctor loss was job related and required to report to MSHA.

Employee was walking to the drill & tripped over a hose causing him to fall forward. EE tried to catch himself on the wooden bench used for seating. The bench rolled over & hit the EE in the rib area on his right side. This caused a fracture to one of his ribs.

Employee had just finished drilling hole and was walking along walkway at top of quarry. He slipped and fell into a void in the rock. Employee fell head first, hitting a rock, knocking him unconsious for 10-15 min. He has serious cut between eyes, possible fracture in nose, swollen right eye and bash in head.

Ee was exiting doorway to t-buck while forklift was making it's way or ramp to doorway. The honking signal was not given in time and the ee was hit by the forks in both knees. one knee (R) is just bruised, (L) badly bruise. xrays show no briken bones althought mri is pending to determine damage to cartilage.

Employee complained of dizzyness as he tried to get off of excavator and lost his balance. Employee fell 4 to 5 feet to the ground cutting openhisarm and hitting his head and shoulder. Employee was planning on fueling up excavator at time of accident.

Employee was using jackhammer when vibrations caused loose cracked rock to fall. He tried to get out of the way, but was too late. One large rock fell and hit him on the elbow, causing swelling and a small cut. EE is awaiting his surgery date for a pinched nerve.

Employee was in quarry/pit standing on a marble block on edge of 5'-6' ditch. The block gave way and employee fell into ditch on another piece of marble. Complained of pain in left foot, left hip, right leg, right wrist and back. Employee was not able to move. Ambulance took to hospital.
