Fall from machine

Employee was climbing down the ladder to the Quarry drill platform when EE slipped and landed on the ground below, which resulted in a right ankle sprain.

Employee was exiting bulk truck, as he stepped on lower step his foot slipped off and he fell to the ground. As he was falling he tried to break his fall and stuck out his arm, breaking his wrist.

Employee was priming blastholes to be loaded that day. While getting ready to prime one hole he noticed a rock on the edge of the hole. He kneeled down to move the rock before he primed the hole, and when he stood up he twisted his knee. Employee was taken to the emergency room where he was told to take 3 days off.

COWORKER WAS BACKING UP F-350 PICKUP WITH CAMPER TO LOAD ANFO BAGS FROM TRAILER. EE WAS GUIDING THE DRIVER WITH A FLASHLIGHT. AS THE DRIVER WAS BACKING UP, EE MOTIONED WITH HIS FLASHLIGHT TO STOP. THE DRIVER KEPT ON BACKING UP, PINNING EE FOR A SECOND BETWEEN THE CAMPER DOOR OF THE F-350 AND THE TRAILER. WHEN THE DRIVER HEARD EE, HE IMMEDIATELY PULLED FORWARD.

EE WAS CLEANING SPILLED MATERIAL FROM UNDER BELT CONVEYER USING A HOE, FELT PAIN IN LOWER BACK. WENT TO E.R. DURING THE WEEKEND, SENT TO CLINIC ON MONDAY 5-12-03, DIAGNOSED WITH BACK STRAIN, PLACED ON MODIFIED DUTY.

EE WAS USING A CHIPPING GUN TO CLEAN OUT THE BUILDUP ON A VIBRATING GREZZLY WHEN THE HOSE ATTACHED TO THE GUN BLEW OFF, CAUSING EE TO LOSE HIS BALANCE AND HIT HIS KNEE ON THE CHIPPING GUN. EE WAS SEEN BY A PHYSICIAN ON 12-30-02 AND WAS DIAGNOSED WITH A CONTUSION, PLACED ON MODIFIED DUTY.

EE WAS ON LUNCH BREAK. WHEN HE HAD FINISHED HE WALKED OUTSIDE THE BREAK ROOM TO STRETCH HIS LEGS. WHILE WALKING AROUND HE STUMBLED ON A ROCK AND HIT HIS HEAD CAUSING A LACERATION REQUIRING TWO STITCHES TO CLOSE. IT HAD JUST RAINED , AREA WAS WET AND SLIPPERY.

EE WAS ON TOP OF OUR BULK TRUCK, LOADING IT FOR THE DAY. WHEN HE CAME DOWN THE LADDER ON THE BACK OF THE TRUCK, AS HE STEPPED DOWN TO THE GROUND HE STARTED TO TURN AND INJURED HIS KNEE.
