Employee was loosening a nut with a wrench when the wrench slipped. The employee lost their footing and fell, the employee used their hand/arm to catch self and injured left wrist

Employee turned to get grab a PVC pipe and when they twisted, they felt a pop and discomfort in their lower back.

Employee was climbing the ladder on a water truck and felt a pop in their left wrist.

Employee had an initial incident on 12/12 which resulted in a lost time incident and the employee had surgery. On 8/14/24 the employee was working normal routine duties when the surgery knee had a numb feeling. The surgeon has advised there is inflation in the area of the surgery and advised the employee not work to rest the knee.

Employee was getting a 6" pipe from the pipe rack when the pipe began to roll off, the employee attempted to catch the pipe and employee's left pointer finger was smashed between the rack and the pipe. The employee was taken to the local hospital for further treatment.

Employee had fly metal imbedded in left upper inner thigh.

Employee was walking on the stripper platform and turned to walk the other direction, when EE did EE felt a pop in the left knee.

Blanks hanging from a crane hook slipped off, employee placed their hand near the load to fix it and a blank fell striking the employee's hand.

Muddy conditions, employee slipped while loading drill steel and employee's pinky finger was smashed between truck bed and steel.

Employee was lifting a blank sheet off of the end of the stripping machine. Initially felt no pain until the next day, reported it 10/31/23.

Lifting/Pushing/Pulling repetitive motion, employee was stripping copper in the tankhouse.

Employees pulling bars from cathode cells, injured employee's index finger was smashed as employee was pulling a bar and the adjacent bar was struck with a hammer by a co-worker. The employee's finger was smashed between the two bars resulting in a fracture.

Employee walking on flat ground twisted ankle, no uneven surfaces or hazards that caused the injury. Employee stated the ankle buckled under employee's own weight.

Employee working in Tyvek suit in hot conditions.

Unsafe use of cutting tools resulting in a laceration to the left thumb.

Repetitive motion resulted in epicondylitis (golfers elbow). Employee was having soreness and pain for approx 5 weeks prior to reporting.

While operating the crash end of the stripping machine a bent blank went over the stepper on one side and became jammed. The employee hit the e-stop on the side to prevent the machine from dragging the stuck blank. The employee tried to dislodge the blank by hand and when it was freed from the jam EE's hand got caught in a pinch point. The employees left middle finger was crushed.

Employees were Staging a 40' Crane Rail against a wall, Injured Employee was struck by the Rail and fractured EE's lower right fibula down by the ankle.

The employee was holding a cathode bar with right hand and using wood harvesting tool with left hand to pry the cathode free from the anode insulator. As the cathode broke free from insulator, the cathode dropped, pinching employee's finger between the cathode bar and the cell bus bar.

Employee injured left shoulder when EE grabbed the railing while exiting a water truck due to worn step grading.

Employee climbing down a ladder felt a pop in right wrist. Employee was given restrictions, due to restrictions employee is unable to do normal job.

Driver was delivering Copper Concentrate to FMI Rail hopper. It was drizzling rain. EE opened the tailgate of the trailer to make sure load was completely delivered. EE slipped and grabbed onto the trailer to keep from falling. EE's middle left finger got slammed in the tailgate resulting in 9 stitches and fracture.

Employee attempted to step over a driscoll pipe that was connected to a pump and slipped hitting EE's chin on the pump.

EE was using a 5lb scraper when EE felt a sharp pain in lower right side of back as EE was making a combination of downward and off to the side motions. EE tried to work the pain out but it wouldn't get better.

777 haul truck operator was jarred around in the cab due to excavator loading the HT made contact with headache rack. Employee was checked and released to full duty. 5/1 employee reported symptoms what was thought to be a personal medical. On 5/11 EE received restrictions may be linked to 4/26. Employee has been referred to a neurologist, results are forthcoming.

Employee was reconnecting pothead to a backup generator. The pothead arched and employee received 2nd degree burns to middle two fingers on the left hand.

Dust fell from above when climbing ladder with attached retractable lanyard

Employee was assigned to stripping starter sheets. A starter sheet was sticking to the blank. Lifting up on the sheet to get a more leverage to pull down again the blank also lifted causing it to come off the hooks. The blank weighing about 50 lbs fell on the top part of employee's foot. The impact caused soreness to the upper portion of EE's left foot. Restricted for 1-day.

Employee pinched right index finger between the gate and the roller while closing the gate causing a laceration that needed two stitches to close wound.

Initially reported FA. Became RD 7/7/17. Employee leaned against handrail that moved. Employee struck hand against the electrical cabinet keeping balance.

Employee got debris in eye during preshift inspection of water truck.

employee was removing HDPE pipe when it slipped and struck employee's finger.

Employee felt pop in stomach. Previous hernia. Released to work no restrictions. Became restricted 2/7 LT for surgery on 2/15.

Muscle strain while picking up heavy wheel chocks

Employee stepped off cell and rolled their ankle on a guard rail.

Employee ran off the road and into a ditch while driving

Employee was removing boots at end of shift when EE felt back pop.

Employee hurt right arm/shoulder swinging a jib clevis on a rope to throw down an embankment

Employee brushed/bumped against a flange on a pipeline in the SX noting discomfort and rash on his right arm. Rash worsened the next day with PA stating the rash may have been aggravated by workplace exposures.

Employee hurt her shoulder and back when she fell while carrying an item. She lost her footing and tripped walking up the stairs. She caught herself with her left hand on the right handrail resulting in her crossing her body/midline

Leach employee began feeling dizzy, experiencing signs of heat related illness, after gathering supplies from connex.

Maintenance employee received 2nd degree burns when the pressurized coolant cap blew off, spraying the employees left forearm. Blistered skin removed using forceps and small scalpel at local medical center as part of treatment. No work restrictions or time lost.

Employee lacerated thumb when he placed it in the line of fire to hold a bolt when it began to slip which resulted in the bolt cutting through his glove, causing a laceration to the left thumb.

Mine employee experienced knee pain while operating water truck 677.

Employee slipped while exiting the Grader on which he was working and slid down 3-4 feet causing him minor abrasions to both elbows and stiffness in middle finger. Emp was x-rayed, cleared for duty. He rec'd surgery consult on right shoulder & assigned no work status 3-15 while on paid investigatory leave for a separate matter

Employee caught middle finger of right hand between starter sheet blank bar and conveyance chain causing laceration.

Employee was spraying down harvested cathodes with a 1" water hose. When he stepped on the hose, he twisted his knee.

Employee was pull starting a diesel sump pump several times during the shift causing muscle soreness

While swinging a hammer right arm began to hurt.

Speed/Haste - Employee failed to maintain three points of contact as instructed. Employee failed to recognize foot was caught in step when dismounting equipment. Employee suffered bruise to torso and strain/sprain in his shoulder. Employee has made full recovery and returned to work.

Removing angle iron holding wooden form in place, the angle iron broke loose pinching EE's finger between the angle iron and a shelf of the wooden form.

While the employee was conducting construction quality assurance for a project at the mine, he walked forward a few feet down the slope and stepped onto articulated concrete block mat (which had some gravel on the surface) and he slipped falling backward and landing on his left shoulder, causing a contusion and ruptured rotator cuff.

On February 24, 2014, employee was diagnosed with carpal tunnel from repetitive motion of stripping cathode starter sheets at the Miami EW Tankhouse. Per employee the symptoms began 12/7/2013.

Employee was operating loader and as he turned steering wheel to the right the heard and felt a pop in his left elbow

Large rock dropped into bed of haul truck and jarred drivers lower back. Driver told supervisor he was fine and left for home. He then complained of soreness at the next shift

Employee was driving a quad at the 27 dump leach to pull 2" lines. Employee drove through a "rip" and strained his back.

Employee was moving a copper starter sheet weighing approximately 12 Lbs. from a moving vertical rack to a horizontal conveyance system and felt pain in the right mid back area. Strain resulted in lost time of one day.

Dozer operator rolled #640 dozer while cleaning narrow road. Dozer went thru the berm at the crest on the 2950 level and came to rest upright on the crest of the 2900 level. Employee was seen and had a full release to work. When seen by company doctor on 9/3/2013, PT was ordered making the incident reportable. There was no lost time and no restrictions.

Dry Hot Day

While shifting forms for a concrete pour, the employee whipped his eye to remove sweat from his brow. While whipping the eye brow he accidently pushed a small flake of metal into his eye without realizing it. It began to irritate him so he informed a foreman who called health and safety. He was transfered to a medical facility to have the flake removed.

Employee was on the stripper line and working to get a starter sheet unstuck. His hand slipped causing his gloved hand to contact the sharp edge of the sheet causing a laceration of his left pinky finger.

Worker was driving the pin for a nuclear gauge with a small sledgehammer, and during the course of the swing, the sledgehammer struck the edge and deflected, causing a shoulder strain.

Employee was cutting radiator hose on 272 HT with a utility knife. The knife slipped and caused a cut on the right index finger.

WHILE WORKING ON A CONCRETE PAD OUTDOORS USING ALL-SOLVE SOLVENT TO REMOVE PROTECTIVE COATING (COSMOLEEN) FROM MATING SURFACE ON THE FINAL DRIVE OF A CAT 793 TO INSTALL REAR TIRES. EMPLOYEE SAID THAT THE WIND PICKED UP CAUSING SOLVENT MIST TO GET IN BETWEEN HIS SAFETY GLASSES AND HIS RIGHT EYE. EMPLOYEE WAITED 2 WEEKS TO REPORT INJURY.

Maintenance mechanics were removing rollers from a track dozer. Mechanic positioned himself approx 12' away while other struck the roller with 16lb sledge. When the roller was struck, a shard of metal broke off and struck the mechanic who had stepped back, striking his right cheek.

Maintenance employee was cleaning a V-seal using a cleaning agent. The employees left hand caught fire as employee was working and the employee sustained 1st and 2nd degree burns on his left hand and forearm.

Employee was injured when the mine passenger van in which he was riding hit a rough spot on the road causing the injured passenger to hit his head on the van headliner.

Haul Truck Driver slipped on muddy ladder platform while descending stairs, spraining his right shoulder while attempting to catch the fall.

Employee was sitting in the operators seat of the 253 haul truck. As he was reaching to close the driver side door, a gust of wind blew the door shut and struck his left hand.

Electrician performing maintenance on 112 shovel was preparing to leave the shovel. He opened the spring latch door to the man-lift and used his hip to hold the spring gate open as he backed into the basket. Below a welder was preparing to board the shovel. The welder started lowering the basket just as the electrician stepped back. The electrician fell to the ground.

While working on a metal plate at the weld shop a small piece of metal came off and got stuck in employee's left eye. Employee flushed eye but was unable to remove the fragment.

Employee was carrying two slings and a clevis to the boom truck. He put the slings on the truck and dropped the clevis on the ground. When he bent over to pick up the clevis, and began picking it up, he heard a pop in his back and fell face down. He crawled over to the truck when his supervisor came over and asked how he was.

Employee was about,10-20 feet away from the 111 shovel as crew was working on replacing teeth on the dipper. One of the crew was using an 8-12 pound sledge hammer on the tooth in order to remove it. The hammer struck metal and a piece flew off striking injured employee in the left elbow.

After loading at the 112 shovel, Haul Truck 262 passed the pole bridge and went over a rough patch of road. The truck was traveling approximately 9 MPH when operator was bounced around, jarring his back.

Lineman climbing wood pole, fell 10'.

Employee was working under wheel well of haul truck when he was struck by a falling piece of mud.

Employee was washing 429 van when the hose he was using got stuck. When the employee flipped the hose to get it loose, he struck the van with his elbow causing a contusion.

EE was jackhammering concrete at the tankhouse when a piece he was working on broke off. The jackhammer fell on the EE's foot. The x-rays showed that the EE's toe next to his big toe was fractured. The EE was restricted to 3 days of light duty.

Employee was working at the lower quickfill. He was removing bad order cable. He bunched up the cable with his green strap and pulled the slack out of the strap. When he finished, he felt pain in his right hand

Employee was moving copper bars out of the way at cell 18. The last bar slipped out of his hand, smashing his right index finger between the copper bar and the bus bar.

Employee bent over to pick up pvc pipe and sprinkler assemblies. He stood up and felt back pain.

EE cut lip with pry bar while removing belt tensioner.

Employee strained his back while placing a wooden block, used for cribbing, into a service truck.

Employee was spraying out the wash bay and tripped over a set of wheel chocks.

Employee was running a loader. It was noticed that he was just sitting in the cab not moving. Supervisor called him on the radio 3 times with no response. Supervisor went to employee and discovered him to be confused and non responsive. Employee was taken to Health Center and was diagnosed as dehydrated.

Employee was changing teeth on an excavator and while trying to remove the pin he struck his thumb with a hammer.

Employee was removing a metal plate from an excavator bucket and it slipped while his hand was underneath the plate

Employee attempted to move track, track links rolled forward pinning hand between roller and links.

While climbing down the machine, employee stepped on shoe string and fell to the ground.

Mechanics helper inhaled the fumes from a can of brake cleaner as the mechanic was spraying a part on the excavator.

Employee slipped and fell while cleaning up spilled hydraulic fluid.

Employee was removing a rock from under a 16" hdpe pipeline with a shovel. Ground had been softened by recent rains. While applying force on the shovel, it rotated, twisting the employee's leg. The twisting force caused the employees kneecap to rotate approximately 45 degrees coming to rest on the ouside of his left leg.

EMPLOYEE WAS ASSIGNED TO INSPECT THE BL2 DISCHARGE PIPELINE FOR LEAKS WHEN THE PUMPING SYSTEM WAS RESTARTED. WHEN CROSSING OVER THE PIPELINES, HE STEPPED OFF OF A 20" PIPELINE ONTO A ROCK CAUSING HIM TO SLIP AND FALL DOWN TO THE GROUND ON HIS RIGHT LEG AND HIP. HE WAS PRESCRIBED PHYSICAL THERAPY ON JULY 19, 2004.

EE WAS OPERATING THE TANKHOUSE STRIPPER CRANE HARVESTING CATHODE. WHEN THE EE PLUGGED CRANE, THE POWER TO THE CRANE TRIPPED OUT. EE TRIED TO RESET THE POWER, BUT COULD NOT. THE CRANE FORCEFULL Y HIT THE CRANE STOP ON THE NORTH END OF THE BUILDING. THIS RESULTED IN DAMAGE TO THE CRANE STOPS AND JARRED THE OPERATOR INSIDE THE CRANE. EE SUSTAINED A LOWER BACK STRAIN.

EE WAS WORKING ON 35 DUMP WHEN HE WALKED DOWN THE SIDE OF 35B TO TRY TO UNBURY 6"PIPE WHEN HE FELL ON HIS LOWER BACK AND BUTTOCKS CAUSING A CONTUSION/STRAIN. THE EE WAS PRESCRIBED PHYSICAL THE RAPY ON 7/24/03.

EE WAS WORKING AT THE KAISER BASIN PUMPING STATION, AND FOUND A DOOR OPEN. WHEN CLOSING THE DOORHE THOUGHT HE HAD HIS RIGHT HAND ON THE WALL, BUT HAD HIS RIGHT THUMB ON THE DOOR JAM. HIS RIGHT THUMB WAS CAUGHT AND EE RECEIVED A LACERATION REQUIRING FOUR SUTURES.

EE WAS CLOSING TAILGATE WHEN THE RT. HINGE BECAME STUCK. WITH PRESSURE STILL APPLIED TO TAILGATE EE REACHED AROUND TAILGATE AND PRESSED HINGE INLINE WITH SLOT. THE HINGE CAME FREE AND CAUGHT F INGER IN PINCH POINT, CUTTING HIS FINGER THAT REQUIRED STITCHES.

EMPLOYEE WAS REMOVING A HYDRAULIC STEERING PUMP FROM HAUL TRUCK #249. THE EMPLOYEE WAS USING A TELESCOPE STAND TO LOWER THE PUMP FROM POSITION. THE PUMP WAS RESTING IN A SADDLE IN TOP OF THE S TAND. AS THEY LOWERED THE PUMP DOWN, THE PUMP FELL OUT OF THE SADDLE AND STRUCK THE EMPLOYEE. THE EMPLOYEE SUSTAINED A LACERATION TO HIS HAND REQUIRING TWO SUTURES.

WHILE TROUBLESHOOTING MOTOR STARTER EMPLOYEE THOUGHT THAT HE HAD RACKED OUT STARTER BEFORE PLACING HIS LOCK ON LOCK OUT. THE RACK DEVICE WAS STILL IN THE "ON" POSITION BUT THE DOOR WAS ABLE TO BE OPENED, THINKING THAT THE STARTER WAS LOCKED OUT THE EMPLOYEE TOUCHED THE INDICATOR ON THE MAIN FUSE, RECIEVING A 2ND DEGREE BURN ON HIS RIGHT INDEX FINGER, RIGHT KNEE AND LEFT HAND.

EE WAS HELPING TROUBLE SHOOT ON #3 BOILER WHEN HE WENT TO REACH FOR THE AUTOMATIC WATER LEVEL BLOWDOWN VALVE HIS FOREARM CAME IN CONTACT WITH A HOT STEAM LINE THE EE SUSTAINED A 2ND DEGREE BUR N.

EE SMASHED HIS MIDDLE FINGER ON HIS RIGHT HAND WHILE PULLING CATHODE BAR FROM STORAGE STANDARD SHEET. THE EE SMASHED HIS FINGER BETWEEN CATHOD BAR AND I-BEAM CAUSING A LACERATION AND FRACTURE REQUIRING THIRTEEN SUTURES.

EE'S HAND BECAME PINNED BETWEEN A BIT SUB AND PUP STEEL WHILE LOADING A BIT SUB. HIS KNUCKLE WAS CUT. ON 2-19-01 HE WENT TO THE DR. DUE TO AN INFECTION INHIS KNUCKLE.

EE WAS DISASSEMBLING TWO SECTIONS OF SCAFFOLDINGWHEN HE WENT TO DISCONNECT ONE OF THE TOP BRACESHE & THE SCAFFOLDING FELL TO THE GROUND WITH THESCAFFOLDING LANDING ON HIM CAUSING A CONTUSION T O HIS LEFT LEG. HE WAS INITIALLY TREATED CONSERVATIVELY. HE WAS REFERRED TO A SPECIALIST. HE HADSURGERY ON 8/1/01.

EE WAS UNLOADING A COMPUTER MONITOR FROM THE BACK OF A PICK-UP WHEN THE EE EXTENDED HIS ARM TO REACH FOR THE MONITOR STRAINING HIS SHOULDER. THE EE WAS PRESCRIBED PHYSICAL THERAPY ON 3-6-01.

EE REPORTED AFTER USING A MSHA RESPIRATOR HE EXPERIENCED SOME SKIN IRRITATION OVER THE BRIDGE OF HIS NOSE AND THE AREA ON HIS CHEEKS THE EE SUSTAINED AN ALLERGIC REACTION FROM THE RESPIRATOR.

EE WAS OPERATING WHEEL DOZER #656 WHEN HE TURNEDTHE AC ON TALCUM POWER WAS BLOWN ALL AROUND THE CAB & ON EE. THE EE HAD AN ALERGIC REACTION THE POWDER & WAS TAKEN TO COBRE VALLEY COMM HOSP THE TALCUM POWDER HAD BEEN SPRINKLED AROUND THE CAB OF THE DOZER TO KILL A BAD ODOR

EMPLOYEE SUSTAINED A LACERATION TO HIS FINGER ON HIS LEFT HAND WHILE PULLING ON A BAR. THE BAR BROKE FREE SMASHING HIS FINGER BETWEEN THE STACK OF BARS AND THE HE WAS PULLIGN ON. THE LACERAT ION REQUIRED SUTURES.

EE WAS LOOSENING A BOLT, AS IT BROKE IT STRUCK A CONDUIT PIPE STRIKING HIS HAND CAUSING A LACERATION REQUIRING SUTURES.

PULLED MUSCLE IN GROIN AREA WHILE ASSEMBLING A FAN IDLER ON A 793 CAT TRUCK.

CHANGING A TIRE ON CAT LOADER, WORKER WAS DISMOUNTING TIRE WITH A RAM 1800 BEAD BREAKER POSITIONED ON BACK SIDE OF RIMBASE. TOOL WAS PROPERLY SECURED, BUT POPPED OUT STRIKING EMPLOYEE IN THE FACE, KNOCKING HIM TO THE GROUND. HAVE REASON TOBELIEVE THAT RIM SECTION FAILED CAUSING THE BEADBREAKER TO SLIP. HE WAS TREATED AT HOSPITAL FOR STITCHES, BRUISES AND BROKEN NOSE.

EE WAS ASSISTING IN STACKING RAILROAD TIES WHILE STANDING DIRECTLY BETWEEN TWO STACKS. THE STACK THAT THE EE WAS FACING GAVE WAY AND A TIE FELL ON EE'S RIGHT LEG PINNING HIM TO THE STACK DIREC TLY BEHIND HIM.

EMPLOYEE WAS REPAIRING SCREEN INSIDE A TANK AT THE SX PLANT WHEN DRY ACID PARTICLES FELL FROM THE CEILING WALLS INTO HIS EYES CAUSING A BURN TO HIS RIGHT EYE.

EMPLOYEE STRAINED HER LOWER BACK WHILE WORKING ON #110 SHOVEL. SHE WAS PULLING ON CABLE WHEN SHE FELT A PAIN CAUSING HER MUSCLE TO STRAIN. THE ATTENDING PHYSICIAN PRESCRIBED PHYSICAL THERPAY O N 03-28-00.

EE WAS REPAIRING EQUIPMENT WITH A TORQUE TOOL/WRENCH WHICH RESULTED IN A PARTIAL AMPUTATION TO HIS RIGHT TOP LITTLE FINGER.

EMPLOYEE STRAINED HIS BACK WHILE UNLOADING TOOLSFROM THE BACK OF A TRUCK. PHYSICIAN PRESCRIBED PHYSICAL THERAPY ON FEB 1 2000.

EE WAS WORKING IN THE MINE DEPARTMENT AT THE FUEL ISLAND WHEN SHE SLIPPED AND FELL CAUSING HER LEFT ANKLE TO TWIST. SHE SLIPPED WHEN SHE STEPPED OFF THE LAST RUNG OF THE FLEX LADDER LOCATED IN THE FRONT OF THE HAUL TRUCK SHE WAS OPERATING.

EE SUSTAINED CONTUSIONS TO HIS KNEES WHEN HE STEPPED ON A ROCK. EE'S KNEE GAVE OUT, HE FELL LANDING ON HIS KNEES, SWELLING OCCURRED ON BOTH KNEES. EE WAS WALKING TO HIS LOCKER BY THE WELD SHOP HE WAS CUTTING A TABLE.

EE WAS HEATING RAIL BORES ON A DRILL, HIS CO-WORKER WAS HAMMERING IN THE RAIL PIN WHEN A PIECE OF THE PIN SHEARED OFF AND STRUCK EE IN THE LEFT FOREARM. HE WAS TREATED AT THE EMERGENCY ROOM, R ECEIVING STITCHES.

EE SUSTAINED A CONTUSION TO HIS LEFT FOOT WHEN THE LEFT OUTRIGGER STRUCK THE OUTSIDE OF HIS FOOT. THIS INCIDENT WAS CLASSIFIED AS A NONREPORTABLE FIRST AID CASE. THE EE CONTINUED TO EXPERIENCE PAIN, DR. REFERRED HIM TO AN ORTHOPEDIST. THE DR. ORDERED ANOTHER XRAY AND FOUND A FRACTURE TO HIS FOURTH TOE.

EE WAS UNLOADING REELER OFF OF A FLATBED TRUCK WHEN HE STEPPED ON A ROCK TWISTING HIS RIGHT ANKLE & FALLING TO THE GROUND. THE ATTENDING DR RX PHYSICAL THERAPY

EE WAS INSTALLING BEARING IN A REAR STRUT WHEN ANOTHER EE SMASHED FINGER ACCIDENTALLY WITH BEARING TOOL.

HE WAS TORQUING BOLTS ON WET DISC BRAKE ASSEMBLY WITH A 3/4" TORQUE WRENCH, PULLING UP ON HALF OF THE BOLTS AND PULLING DOWN ON THE OTHER HALF (AROUND 30 TO 40 BOLTS).

WHILE HELPING PUNCH CATHODES, EMPLOYEE WAS HOLDING A CLAMP. THE CLAMP FELL TO THE GROUND PULLINGTHE EMPLOYEE. HE SUSTAINED A STRAIN TO HIS LOWERBACK. TREATING PHYSICIAN PRESCRIBED PHYSICAL THE RAPY

EE SUSTAINED NECK AND SHOULDER STRAINS WHEN THE BUCKET OF #110 SHOVEL OPENED PREMATURELY AND STRUCK THE BACK OF THE HAUL TRUCK #262 AS SHE WAITED TO BE LOADED.

A MINE EQUIPMENT OPERATOR OVERTURNED CATERPILLAR HAUL TRUCK #254 WHEN HE POWERED THROUGH THE BERM AT THE EDGE OF THE PAD HE WAS DUMPING OVER. EE SUSTAINED SMALL FRACTURES IN HIS LOWER SPINE AR EA.

EE LIFTED A PIECE OF METAL TO SET IN PLACE.

EE WAS CLEANING LEACH PADS, AT THE SX DEPT., SOLUTION SPRAYED INTO HIS RIGHT EYE CAUSING AN EYE INFECTIN

EE WAS MAKING REPAIRS TO A LEAKING WATER PIPE THE PIPE AREA HAD BEEN EXCAVATED TO EXPOSE THE PIPE. WHEN THE EE STEPPED INTO THE HOLE THE SIDE COLLAPSED TWISTING HIS KNEE, AS HE CAUGHT HIS BA LANCE. CLASSIFIED AS A 1ST AID UNTIL MENISCUS TEAR WAS IDENTIFIED, SURGERY PERFORMED ON 7/22/98 REPORTING OVERSIGHT FOUND DURING INTERNAL AUDIT

WHILE TRYING TO PRY A BLOCK BACK UP AFTER SLIPPED, HIS PRYBAR ALSO SLIPPED AND WHEN HE FELL HE SMASHED THE LEFT RING FINGER BETWEEN PRY BAR AND A PIECE OF METAL.

WHILE CHANGING WHEEL MOTOR ON 930 E AND RUNNING IMPACT WRENCH, SOMETHING FLEW INTO LEFT EYE.

A OIL SPILL OCCURED SO HE GRABBED A 55 GAL. DRUM THAT HAD ABOUT A 1/4 DRUM OF FLOOR ANY LEFT. HE WENT UNDER TRUCK AND STARTED POORING FLOOR. HE FELT A PULL AND PAIN IN LEFT LOWER BACK AREA.

EE WAS IN THE HAUL TRUCK WAITING TO BE LOADED WHEN THE SHOVEL OPERATOR SWUNG THE SHOVEL AROUND AND THE BUCKET STRUCK THE HAUL TRUCK; EE SUSTAINED A BUMP ON THE HEAD AND A NECK STRAIN.

EE'S RT MIDDLE FINGER WAS CUT WHEN IT WAS PINCHED BETWEEN A WELD HEAD AND A HORSESHOE PLATE DURING REMOVAL OF THE HORSESHOE PLATE. SUTURES REQUIRED.

EE SLIPPED OFF TRUCK BOARDING LADDER, FELL TO THE GROUND & SPRAINED HIS LEFT KNEE.

EMPLOYEE WAS ATTEMPTING TO TURN DRILL SUB TO BRING DECK FORK OUT. HE TWISTED HIS BODY AND FELT SOMETHING PULL IN HIS BACK.

WHILE REMOVING A 18,000 GALLON EMPTY WATER TANK.. EE WAS CUTTING A BUSHING OFF OF THE REAR TANK MOUNTS, WHEN THE BUSHING WAS CUT LOOSE THE TANK SHIFTED ABOUT 4" AND SQUEEZED EE BETWEEN THE T ANK AND THE TIRE. THE TANK WAS SUPPORTED BY THE CRANE. EE RECIEVED BRUISING.

EE WAS RESEALING TRUCK WHEEL. HE WAS ON TOP OF DRAIN CAR CARRYING WHEEL BEARING. GRATE ON CART FLIPPED UP AND HE FELL BACK TWISTING HIS LEFT KNEE.

HE WAS INSTALLING A RADIATOR IN A TRUCK. HIS RIGHT FOOT WEDGED BETWEEN THE PULLEY AND WATER TUBE. HE MOVED HIS FOOT AND FELL HITTING HIS CHEST ON THE OIL COOLANT REGULATOR. HE ENDED UP HANGING FROM REG HOUSING BY HIS HANDS.

EMPLOYEE WAS PERFORMING CONCRETE/WATERPROOFING REPAIRS AND WAS USING A GRINDER.GRINDER SLIPPED AND CUT EMPLOYEE IN THE KNEE.

EE CLAIMS HE EXPERIENCED LOWER LEFT ABDOMINAL PAIN WHILE HE WAS PULLING A WRENCH ON A BOLT. SURGERY SCHEDULED ON 10-4-96. STATUS CHANGED TO REPORTABLE/LOST TIME.

EE STRAINED LOWER BACK WHILE LIFTING AN ANODE. BECAME REPORTABLE ON 12/27/96 WHEN EE COULD NOT ADEQUATELY PERFORM RESTICTED WORK DUTIES, NOW A LOST TIME.

EMPLOYEE'S TOOK A 793 HAUL TRUCK TO THE WASH RACK TO CLEAN THE REAR AXLE HOUSING AREA. INJURED WAS PUTTING THE HOSE ON TOP OF THE TRUCK TO CLEAN THE TOP. WHEN HE LAID THE HOSE DOWN THE VALVE O PENED AND THE WATER PRESSURE CAUSED THE HOSE TO WHIP BACK AND THE VALVE STRUCK EMPLOYEE ACROSS THE MOUTH AND NOSE. HE WAS WEARING HARD HAT AND SAFETY GLASSES.

EE WAS PUTTING A CABLE CLAMP ON A CABLE TO KEEP AN ELECTRICAL CABLE FROM DROPPING. WHILE HE WAS DOING THAT THE CABLE WAS STRETCHED ACROSS THE ROAD. A TRUCK STRUCK THE CABLE, CAUSING A CABLE WI NCH TO STRIKE HIS HEAD.

EE WAS USING A PRYBAR TO PRY ON AN AXLE BUSHING, THE PRYBAR SLIPPED AND STRUCK HIS MOUTH, CUTTING HIS LIP AND CHIPPING HIS TOP, FRONT, LEFT TOOTH.

THE EMPLOYEE WAS INSTALLING AN INSULATOR ON A LEAD ANODE. THE ANODE SLIPPED AND CAUGHT HIS FINGER BETWEEN THE ANODE AND THE TANK LEDGE.

EMPLOYEE WAS STEPPING OFF WATER TRUCK LADDER ONTO GROUND AND TWISTED HIS ANKLE.

EE BACKED HAUL TRUCK INTO THE SHOVEL STAGING AREA TO BE LOADED. THE SHOVEL SWUNG INTO THE TRUCK AND STRUCK THE BED WITH THE SHOVEL. THIS ACTION CAUSED THE TRUCK TO JAR AND SPRAINED THE EE S NE CK.

WHILE USING A 18:1 TORQUE MULTIPLIER TO TORQUE A LEFT SUSPENSION ON A HAULAGE TRUCK, THE 1' DRIVE ADAPTER BORKER CAUSING THE TORQUE MULTIPLIER TO STRIKE EE ON HIS LEFT JAW. HIS JAW IS BROKEN.

EE STATES WHILE PUTTING JAMS IM FOOT CLAMP, HIS FOOT SLIPPED. CLAMP CAUGHT HAND, SUSTAING PARTIAL AMPUTATION OF LEFT INDEX FINGER.

EMPLOYEE WAS MOVING THE TRANSFER WAGON AND WHEN HE GAVE THE WAGON A "FORWARD" COMMAND ON THE LOCAL CONTROL PANEL THE WAGON RESPONDED BY MOVING IN REVERSE. THE REVERSE MOTION CAUGHT HIS RIGHT E LBOW BETWEEN THE WAGON AND A BEAM.

EMPLOYEE CLAIMS HE WAS PERFORMING MAINTENANCE AT THE #34 DUMP CURE PAD. HE BENT OVER TO DO SOME REPAIR WORK AND SANK INTO THE MUD, LOSING HIS BALANCE AND TWISTING HIS ANKLE. HE SUSTAINED AN AN KLE SPRAIN.
