Loader operator in 980F was moving AB from stacker area to stockpile and hit a bump or dip in surface at 15 MPH, which rocked loader hard, causing EE to jerk in his seat, straining his lower back. EE was wearing seat belt, shock absorber working properly in loader.

Performing a belt splice repair, he used a short pry bar to lift flexcos. Bar slipped off flexco lacerating little finger, left hand. Employee was wearing gloves. Laceration required six sutures. No lost time. No impairment associated with accident.

OBSERVATION DECK OF JAW CRUSHER. ROCK WEDGED IN JAW, HYDRAULIC BREAKER MOVED INTO POSITION. BREAKER SLIPPED OFF ROCK, PINCHING HAND OF OBSERVER BETWEEN GUARD RAIL AND HAMMER.
