Performing a belt splice repair, he used a short pry bar to lift flexcos. Bar slipped off flexco lacerating little finger, left hand. Employee was wearing gloves. Laceration required six sutures. No lost time. No impairment associated with accident.

HE WAS UP ON THE BIG JAW CRUSHER ABOUT 10 FEET OFF THE GROUND. HE WAS WELDING AND AS HE WENT TO MOVE TO A NEW LOCATION. HE LOST HIS FOOTING AND FELL. HE LANDED ON HIS HEAD AND SHOULDER AND LOS T CONCIOUSNESS FOR A FEW SECONDS.
HE WAS CUTTING A BELT WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT HIS RIGHT LEG ABOVE THE KNEE. HE RECEIVED STITCHES AND THEN RETURNED TO WORK.
