While changing flashing on a conveyor using a hammer and punch for bolt holes. EE hit his left hand with the hammer.

Over-exertion in lifting objects

Jaw crusher was plugged while cleaning rocks out strained his lower back. EE did not report pain in his back unitl 8/21/08.

While changing bucket teeth on loader, tooth fragment broke loose and went into left thigh.

Helping change crusher liner holding onto tag line and stepped between crusher and catwalk area. **Employee reported to plant manager and they determined no injury. On 7/17/07 the employee's elbow was swollen & he was sent to the clinic and fluid was removed.**

EE WAS CLEANING GRIZZLY ON JAW FEEDER WITH A 8 LB HAMMER. THE HAMMER CAME IN CONTACT WITH THE UPPER BEAM SUPPORT AND STRUCK EE ON LEFT SIDE OF HIS HEAD.

WHILE REPLACING BELT AND TAIL PULLEY ON MP02 CONVEYOR. THE EE WAS HELPING LEFT THE TAIL PULLEY INTO PLACE WHEN THE TAIL PULLEY SLIPPED AND CAUGHT HIS HAND PUSHING IT AGAINST THE FRAME.

WHILE TIGHTENING DRILL BIT WITH SCREW DRIVERS RIGHT HAND SLIPPED AND CUT LITTLE FINGER ON DRILL BIT. REQUIRED C9 STITCHES TO CHOSE WOUND.
