Helping change crusher liner holding onto tag line and stepped between crusher and catwalk area. **Employee reported to plant manager and they determined no injury. On 7/17/07 the employee's elbow was swollen & he was sent to the clinic and fluid was removed.**
EE WAS CLEANING GRIZZLY ON JAW FEEDER WITH A 8 LB HAMMER. THE HAMMER CAME IN CONTACT WITH THE UPPER BEAM SUPPORT AND STRUCK EE ON LEFT SIDE OF HIS HEAD.

AFTER CONE CRUSHER ADJUSTMENT WAS COMPLETED THE MACHINERY WAS PUT INTO A SINGLE PHASE STARTUP. EQUIP WAS THEN TURNED OFF, ELECTRICAL PANEL WAS OPENED ON BACKSIDE WITH DISCONNECT STILL ENERGIZE D. KNIFE EDGE ON DISCONNECT WAS PUSHED IN WITH A SLEDGE HAMMER. THE HAMMER TOUCHED TWO PHASES OF 480 VOLTS TOGETHER RESULTING IN FLAMES TOWARD EE. ELECT. PANEL WAS DESTROYED.
