Employee was holding a wedge used to remove a stuck sheave pulley on a motor shaft. Another employee was striking the wedge with a 4lb. hammer and the hammer missed the wedge and struck holding employee in top of left hand. Employees were wearing full PPE, but hammer strike resulted in fracture of left hand.

Employee was assisting with supplies in the area where two welders were welding. Employee stated they did not look directly at the welding light but due to being in area while welding was taking place, the reflection of the welding light caused their eyes to be subjected to arc flash.
Employee picked up a piece of flat metal off back of welding truck. When employee began to carry metal to the work area, they felt an intense pain in their abdominal region. After medical review, employee was diagnosed with an abdominal strain.
Employee was in haul truck being loaded with material in pit. Loader was dumping material in bed of haul truck when a large boulder dropped out of loader bucket into haul truck bed and caused haul truck to shift abruptly. Thus, resulting in a strain to haul truck operator's neck and shoulders.

Temp worker was un-bridging rock from C4X front feeder. After they un-bridged the feeder and was standing to the side and observing the feeder, a rock rolled off the conveyor belt and hit temp worker in left leg. The temp did not seek medical attention and finished shift. The next morning 11/8/2022, temp went to dr. and returned to work on light duty.

EE was changing a bearing. The bearing housing slipped and smashed EE's left ring finger. EE was a temp and EE's assignment was ended on 6/8/2022 because EE didn't follow instructions on going to the medical facility to have the injury looked at.
EE was cutting on a conveyor belt with a pocket knife slipped and caused the knife to fold cutting EE's left index finger. EE had 4 stitches. EE didn't use the proper tool to cut the belt, so EE was let go on 3/25/2021. EE was working through a temporary service.
Employee was putting in feeder bottoms and twisted then back started hurting. Employee didn't want to go to the doctor, so they waited till today 9/3/2020 for us to take them to the doctor.
EE was removing a guard on the counter weight of C4X when the guard pinched right hand middle finger.

Lowering a ladder into the gyro to check worn concaves and didn't notice that the safety cable had tangled around his ankle and he released the ladder into the gyro causing him to lose his balance and hurt his left knee.
