Caught in, under or between a moving and a stationary object
EE was changing a tail pully bearing. EE was holding the shaft up with a bar while cleaning it for reassembly. When bar slipped off of the shaft and pinched left index finger between shaft and conv. frame.
Fall from machine
employee was tightening an oil filter on a front end loader when EE slipped off the walk way/battery box and fell to the ground.

Employee was cutting a doorway in a cargo container with a hand grinder. The steel pinched the wheel and the grinder slipped hitting EE in the leg requiring 5 staples.
