Miner was attempting to deflect material from a moving conveyor belt with a shovel. The shovel was caught by the conveyor causing the miners hand to become stuck betwen the shovel and the frame of the machine. The miner was wearing a wedding ring which caused most of the injury. This consisted of laceration to the finger and avulsion of the skin.

Fall from ladders

EMPLOYEE JUMPED FROM LADDER TO SLING BINS THAT WERE STACKED ON FLOOR INSTEAD OF COMING DOWN A FEW MORE STEPS.

Caught in, under or between a moving and a stationary object

Forklift was being driven, unloaded, back to the loadout area. The pavement had been wetted to suppress dust. Employee was traveling too fast, swerved to miss a pallet in the line of travel. Forklift spun 120 degrees. His right rear wheel slid into ditch and the left rear side of the forklift climbed a berm along side of ditch. Forklift overturned on its right side.

Struck by flying object

HAMMERING A BEARING POWER SCREEN, PIECE OF SLAG FLEW OFF HITTING MINER & LODGING A PIECE INTO RT. SIDE OF CHEEK REQUIRING BUTTERFLY STITCHES. MINER WAS WEARING SAFETY GLASSES & HARD HAT.

WAS LIFTING A ROCK (150 LB) WITH CO-WORKER WHEN FINGER GOT CAUGHT UNDER ROCK. PULLING HIS HAND OUT CAUSED MULTIPLE LACERATIONS TO LEFT INDEX FINGER. EE WAS GIVEN A TETNUS SHOT.

WALKING DOWN HILL, SLIPPED AND FELL. CUT HAND ON A PIECE OF SLATE.

Struck by falling object

CUTTING DOWN TREES AND A DEAD TREE FROM APPROX 10 FT AWAY FELL DOWN ON HIM CAUSING BRUISING TO SHOULDER. DR ADVISED HIM TO TAKE 1 WEEK OFF WORK.

WAS TIGHTENING THE JAWS ON CRUSHER WHEN ROTATIONPULLED WRENCH OUT OF EMPLOYEES HAND. WRENCH SMASHED TOP OF FOOT.

EE WAS WALKING AT THE REAR OF THE MILL AND TRIPPED ON A PIECE OF ANGLE IRON & FELL.

Fall from machine

EE WAS ADJUSTING JAW SCREEN AND MOVING ALONGSIDE EQUIPMENT WHEN HE LOST HIS BALANCE. HE REACHED OUT TO STEADY HIMSELF AND GRABBED A GUARD WHICH WAS INSECTURE. HE LET GO AND FELL FOUR FEET TO T HE GROUND. RT. UPPER ARM, RT. ROTATOR CUFF STRAIN.
