Employee was cleaning using a Bobcat MT55 (walk behind loader). Employee became pinned between Bobcat machine and belt frame. Torso and upper legs were pinned.

Employee was operating a 972M Loader. Employee felt something wet on arm, and saw was bleeding from back of right arm, just above the elbow. Employee discovered a small puncture, but has no idea what caused it. Couldn't get bleeding to stop, so went to clinic. A stitch was used to close wound.

Cleaning threads from wire wheel, chunks of wire from wheel came off and some got behind safety glasses into right eye.

Cleaning a chute with a metal bar. Scraped left ringer finger knuckle across chute. Cut became infected.

Cutting shock tube when knife slipped off tube and caused a 2cm laceration to left thumb.

Employee leaned ladder against plastic tank, got on ladder, and ladder slipped down the tank. Employee suffered strain to right wrist and elbow.

Stepping down from access walkway. Twisted right foot on rock. Strain to right foot. X-ray negative for fracture.

Moving product chute, ratchet mechanism came off thread and fell on right hand. Laceration and swelling to right hand.

Cleaning around the wash plant with walk-behind Bobcat. Walked backwards into crusher part beside crusher, pinning left leg between crusher part and Bobcat.

While carrying a 5-gallon bucket of paint up 3 flights of stairs at secondary plant, employee ""felt something pop"" and felt instant pain in groin area.

Taking small pieces of wood out of 1x4 stockpile. Crossed up feet, and fell down, hitting shoulder and head.

Clearing brush and trees from hillside, came into contact with Poison Oak. Employee is allergic apparently. Poison Oak has spread to various parts of his body.

Grinding chisel for use on F250, got metal grindings in right eye, past safety glasses and face shield.

Worker was putting panels on the racking system and hit his head on a cross rail. EE was wearing hard hat when the accident happened.

Jammed pinky finger while installing a v-belt resulting in a dislocated finger.

Worker was adjusting frames, he tripped lost his balance and fell on a frame.

While cleaning chute, he got the end of finger between chute and bar.

Cleaning around plant with shovel.

He used his foot and leg against a wrench to free a bold & felt pain in his knee

Contact with Poison Oak

Reeling of water hose, felt pain in left shoulder.

Cleaning chute at plant. Smashed tip of little finger between chute wall and bar on 1/14/08. Reported to manager on 1/16/08.

While dismounting backhoe, EE stepped on uneven ground, thereby spraining right ankle.

Running ""walk-behind"" compactor near 4 foot embankment which sloughed and pushed employee into machine and then to the ground, pinning his right leg and arm.

COWORKER ASKED EE TO HELP HIM STACK STRAW BALES ON UNEVEN GROUND. THEY STACKED BALES 5 HIGH. ONE BALE FELL OVER AGAINST EE'S UPPER LEFT ARM, CAUSING HIM TO FALL TO THE GROUND. EE ASSURED HIS SUPERVISOR THAT HE WAS NOT INJURED AND RETURNED TO HIS SHIFT. THE NEXT DAY EE COMPLAINED OF BACK PAIN, AND WAS SENT FOR MEDICAL EXAM.

WORKER WAS DRIVING WATER TRUCK DOWN A NEWLY CONSTRUCTED ROAD FROM 5TH TO 4TH BENCH AT QUARRY. TRUCK SLID TOWARD EDGE OF ROAD. WORKER KEPT DRIVING TO TRY TO CORRECT SLIDE BUT WAS UNABLE TO. TRU CK ROLLED OVER EDGE OF ROAD AND SLID DOWN TO NEXT BENCH. WORKER INJURED RIGHT ARM WHEN TRUCK ROLLED OVER.
