Standing on edge of berm turned to walk back down to road right boot stayed planted in soft material as rest of body turned knee popped.

Employee was driving water truck on feed deck wetting down stockpile. He backed up and started spraying again when the left front tire hit a rock and spun the steering wheel sharply, jamming his right wrist before he could let go of the wheel.

Employee was recording hour meter and checking fire ext on GR113 when EE went to climb down off grader his foot slipped off step, fell backwards to the ground landing on molboard of machine.

AN EMPLOYEE THREW A USED 4 1/2"" GRINDING DISK OFF AN ELEVATED PLATFORM & THE DISK BOUNCED OFF A WELDING TABLE & STRUCK ANOTHER EMPLOYEE IN THE RIGHT EYE. EE WENT TO THE E.R. & WAS INFORMED HE HAD A SCRATCH ON HIS EYEBALL, APPROX. 3/4"" X 1/16"" & THAT HE HAD A PARALYZED PUPIL.

EMPLOYEE'S WERE TRYING TO DISLODGE LARGE ROCK FROM 42"" JAW CRUSHER FEED OPENING. INJURED EMPLOYEE STRUCK HIS FINGER WITH SLEDGE HAMMER

EE WAS IN PRONE POSITION WHILE SCARFFING EXISTING TAC WELD - HOT SPRAY - SLAG - BOUNCED OFF STEEL STRUCTURE AND ENTERED EAR VIA BACK OF PROTECTIVE WELDING HOOD.

EE WAS CHANGING MESH OR SIZING SCREEN - HE ATTEMPTED TO CROSS TO OPPOSITE SIDE OF SCREEN DECK AND SLIPPED DURING TRANSITION. HE FELL (SAME LEVEL) ON SCREEN METAL FRAME PUNCTURING HIS LEG. EXISTING WALKWAY SHOULD HAVE BEEN USED.

THIS INCIDENT WAS CAUSED BY EE'S OWN ACTIONS, A STRAIN TO HIS UPPER NECK.

EMPLOYEE WAS DISMOUNTING F.E.L., STEPPED ON A 3-4"" ROCK AND TWISTED RIGHT KNEE.

USING A RAZOR KNIFE FOR PLANT MAINTENANCE, WAS CUT BY THE KNIFE.
