Employee was driving water truck on feed deck wetting down stockpile. He backed up and started spraying again when the left front tire hit a rock and spun the steering wheel sharply, jamming his right wrist before he could let go of the wheel.

AN EMPLOYEE THREW A USED 4 1/2" GRINDING DISK OFF AN ELEVATED PLATFORM & THE DISK BOUNCED OFF A WELDING TABLE & STRUCK ANOTHER EMPLOYEE IN THE RIGHT EYE. EE WENT TO THE E.R. & WAS INFORMED HE HAD A SCRATCH ON HIS EYEBALL, APPROX. 3/4" X 1/16" & THAT HE HAD A PARALYZED PUPIL.
