Employee was pushing pit run material to the feeder in a D10. Employee states struck a rock and this jarred EE around in the seat. EE stated that it started hurting later that day. *Injury was not reported by employee until 11/2/2023.
Removing welder from service truck. Slide tray slid all the way out of compartment. Welder fell on employee's foot, landed just past edge of the steel toe. Broken right big toe.
Employee pinched right index finger between fork and mast while adjusting fork spread on forklift. 4 stitches to close laceration.
On pit pump in main pit to remove float and pump from service. Needed to disconnect from discharge hose and the float flipped in the water. Employee fell into the water and hit his knee as he was going in and cut his leg requiring stitches.
Employee was installing a wear plate on a conveyor that feeds a twin shaft coarse material washer. He was standing on a partition between the two tubs. He removed his gloves to retrieve hardware. At this point, he lost his balance and fell aproximately 3'. He braced his hand during and cut his left hand in between the index and middle finger that required 10 sutures.
CRANE WAS LOWERING SCREEN INTO SCRUBBER. EE WAS TRYING TO POSITION SCREEN, IT SLIPPED & CAUGHT HIS FOREARM BETWEEN SPRAY BAR IN SCRUBBER & SCREEN, CUTTING WRIST.
WHILE CUTTING METAL IN TUNNEL, A PIECE OF HOT SLAG FELL INTO RIGHT EAR.
THE FIRST WE KNEW OF THIS INJURY WAS ON 9-13-2001. EE IS NOT SURE WHEN IT OCCURRED OR WHAT ACTIVITY HE WAS PERFORMING. THE MORNING OF THE 13TH HE NOTICED A BUILGE IN HIS ABDOMEN BEFORE COMING TO WORK. HIS WIFE CALLED HIS FAMILY DR. WHO SAID HE SHOULD GO TO ER. THE DRS THERE SAID IT WAS 3WORK RELATED. HE WAS ALSO SEEN BY OUR CO DR. & A HERNIA SPECIALIST WHO ALL AGGREED IT WAS WORK R
REACHING INTO CAR TO GET TOOLS, A RAZOR KNIFE CUT EMPLOYEE'S HAND.
AS EE STEPPED UP PORTABLE BASE PLATFORM, HE SLIPPED AND FELL TWISTING AND INJURING FOOT.
EE WAS WALKING BETWEEN 2 SCREENS AND SLIPPED HITTING HIS FACE AGAINST CONVEYOR.
WHEN CLOSING EQUIPMENT DOOR, EE'S HAND SLIPPED INTO JAM AND DOOR CLOSED.
