Injured worker was working on hopper underneath the 6x16 screen. The job was welding patches. EE had EE's back leaning against the wall with one leg extended and the other one bent while welding. When the job was done, EE extended EE's legs, and EE's knee was numb. And started feeling like EE pulled something on EE's back.

The pin on the rental excavator was walking out the wishbone, operator was holding the pin in place while maintenance guy was trying to realign the bearings. The mechanic asked the operator to pull the pin out a little and they pulled it all the way and that caused fingers to stick out of the hole causing the very tip of the maintenance man's finger to get cut.

Struck by flying object

When knocking the wedge tooth off with a hammer, the corner the tooth broke off shooting through employee's overalls into EE's abdomen skin.

Employee was using hammer on conclave liner to remove it. EE placed EE's foot under the liner and was injured.

The employee was cleaning the under flow pipe while working in the mud tank. Allegedly developed rashes on hands 16 days after contact with the mud. Employee did not lose any time until 03/02/2015 when we received a letter from Arrowhead Orthopedics advising us about his restrictions.

When cleaning a shovel tooth from telsmith jaw ee asked for a shackle. Another ee dropped one down in the feeder ee tried to catch it and cut his finger.

Cleaning electric shaft with brake cleaner. Lighting the torch causing the fumes to flash. The flash caused minor burns to EE's face. (Looks like sunburn).

Employee stated that he hurt his shoulder in the water truck. He hit rock and jerked the wheel causing a shoulder injury.

Standing over top of conveyor - bending over to remove troughing rollers - twisted back wrong. Reported as incident only on 8/15/12 (1st aid). Continued working until 8/22/12 - back hurting more.

EE was attempting to remove a keeper pin on the side of the bucket with a chipping hammer when a sliver of metal came off and lodged in the upper part of his right hand.

Santa Ana winds were blowing. We were loading shot with windy conditions. Loaders were digging below us creating more dust - got dust in eye.

Employee was moving portable light plant with #270445 service truck (light plant had broken jack that would not retract or extend)When lifting tongue of light plant off of truck hitch, light plant shifted towards employee smashing left hand thumb on bumper of truck.

Employee was moving a suction hose & pump in the pit area with a forklift. Hit a low spot in the pit area and the forklift leaned over onto two wheels. He jumped from forklift spraining his ankle.

Repairmen were changing out a crusher. The hand rail on the work platform had been removed so the the crane could lift out the crusher. EE stepped off the platform while the temporary rail was being prepared. He was hurt while trying to break his fall.

While using a cutting torch on steel of primary crusher (wearing goggles), slag flew up under goggles and got into left eye.

Repairing crusher, he jumped from conveyor to catwalk causing left knee strain and contusion.

Employee was hit by fly-rock while looking for a truck to load. He was walking on the driveway of the parking lot east of heavy equipment shop. Incident occured on company owned property.
