SEE ATTCH: DRIVING LOADER TO PIT TO REMOVE CLAY OVERBURDEN PREVIOUSLY PUSHED TO FLOOR FROM TOP OF PIT BY TRACK DOZER.WHEN EMPL STARTED DESCENT INTO PIT LOADER ROADE TOE OF BERM FOR SEVERAL HUN DRED FEET THEN VEERED AT 45 DEG ANGLE OVER BERM OVER 50-60' AREA BETWEEN BERM AND PIT WALL THEN DROVE OFF EDGE OF PIT LANDING ON PIT FLOOR.ALTHOUGH LOADER WAS FOUND IN UPRIGHT POSITION CRUSHE
