Mechanic was in the process of changing the teeth on an excavator bucket. The teeth were difficult to remove and required the use of a sledge hammer to work them loose. While hammering the one of the teeth loose, a small shard of metal broke away on impact and imbedded into employee's left leg below the knee.

EE was finishing a conveyor belt repair & was climbing off the conveyor when left foot got caught by the water hose that runs along the frame of the conveyor causing EE to trip & EE landed on right knee. EE reported the incident to management, first aid was administered and the employee was taken to clinic for evaluation. EE was placed on modified duty.

Employee was working in the tunnel around the surge feeder on the platform and had left the trap door open. EE was spraying down the feeder with water when EE had sprayed self in the face and stepped back into the access point falling less than 6' onto the dirt below. One of the tail pulleys had buildup behind the guard that was covering the wing pulley. EE took a 36" long rod with a hook at one end and inserted it in a 1" space in the guard to sweep the material free from the tail pulley. In doing so the rod was caught by the wing pulley and ripped the rod from his hand which resulted in breaking the skin and his thumb.

We were cutting down a 30' section of conveyor and IE was making what he thought was the final cut, but he over looked one 2" metal water pipe on the catwalk so when he made the cut, the 2" metal water pipe came up and hit IE from behind and injured his back and face.

Employee was washing out the main dozer trap feeder with a water hose. When he exited the feeder he slipped on the wet rock and cut his right hand on a sharp rock. He was not wearing gloves at the time of the accident. Went to doctor and received three stitches.

Emp. was checking PC-6 head section drive belts, 14" from ground. He climbed onto belt, got on his knees, leaned out over the box, he grabbed a piece of angle iron that was not connected and slipped away. He lost his balance and fell into head box (5 1/2" deep). He reached up with his left hand & grabbed side of head box to break his fall, dislocating his left shoulder.

On 10-16-09, 8:30 a.m. he reported to superintendent that his back and leg pain was worse than yesterday (did not go away overnight). Employee was sent to medical clinic. Emp was starting load out system, slipped on concrete pad falling into 3 feet of soft material, he managed to free himself. He went to mmedical clinic and received a return to work on 9/5/09. He received pains and anti-inflamatory medications. Upon follow up 9/8/09 he was given physical therapy for 2 weeks 3 times a week. He returned to full time work with no loss time.

On 8-6-07 employee complained about sore knee from slamming on D9L Dozer brakes. Did not want medical attention. On 9-4-07 complained about his knee being swollen. Sent him to doctor and he was cleared for work. Medication: Oxyproxin more than 1 dose (anti-inflammatory).
