While flopping the (metal) "material fingers" the employee right index finger was pinched and while trying to remove his right index finger his left thumb was crushed on the left side.

Porterville Rock and Recycle Inc · Fall from machine

Employee was washing the floor deck of a 355 Komatsu. Once finished he stepped down to the track and slipped with his right foot. His left foot became wedged in between the blade push beam and track frame as he fell from the top of the track to the ground level. The employee was immediately taken to the local emergency room and determined that he had broken his left ankle.
