The miner had a reportable standard threshold shift on their annual hearing test. A follow up test confirmed the shift.

Rock Structures Construction · Struck by falling object

Loader operator improperly positioned loader bucket while replacing a worn out tooth. Tooth slipped off bucket during installation and fell about 30" onto the top of operator's foot behind steel toe of boot. E.R. doctor diagnosed severe bruise and broken blood vessel in foot. One week off and re-check.
