miner was driving 1989 Kenworth water truck up the haul road and either shifted or stalled the water truck the truck began rolling backwards rolled into muddy conditions truck began sliding and overturned. Employee climbed out of water truck and complained of some low back pain.

Employee was cleaning hammermill and stepped on rock causing right ankle to twist.

Employee standing next to handrail by prep screen on flat ground and went to turn to walk in the opposite direction and twisted knee.
The employee was pushing a wheel barrow full of dirt/gyp and stepped wrong on his right foot causing it to twist.

The employee was using a come along to draw in dozer track frame to line up and insert pin for track. The come along chain broke hitting the employee in right side of jaw. He received a small cut on ouside of jaw and inside of mouth and it knocked a tooth loose on lower jaw.

Injured person reported at 3:00 p.m that at approx. 7:45 a.m. he was shoveling and his back started hurting. He continued to work and then later got off of his escavator to move a rock and he felt something pull in his lower back.

Moving pin to to align and put in excavator, the pin grazed up against employee's left knee/shin area.
employee cut right middle finger required two stitches and tetnus shot returned to work same day no lost time
Stepped off small berm by conveyor 9 and hurt lower back.
Employee climbed up to get into the dozer (to begin operating it) and twisted right knee
Employee states he was exiting the time clock room and stepped wrong on his left foot. He has multiple fractures on top of left foot.

Employee was sitting down in control tower, started experiencing extreme pain to lower abdomen right side.

Employee was re-tarping in the plant area. A piece of baling wire used to tie off the tarp poked him in the right eye.
Employee was driving a 1991 325M Dresser empty up to the top of the mountain to get a load of Gypsum Rock. The road was too wet and the truck slid down the haul road in reverse rolling into a burm and rolled on its side.
Employee was cleaning the hammermill when a co-worker stepped on the bearing causing one of the hammers to fall on the top of his left hand. Employee had a small abrasion on the top of his left hand causing tenderness to the top of his hand, two fingers, wrist and upper forearm.
Getting out of the Water Truck and stepped on foot wrong
Dust flew into employee's left eye This was not a lost time accident the dr looked at his eye administered eye drops and wrote out a prescription for eyedrops. EE returned to work no restrictions immediately. No Follow up visit to the doctor is required.

Employee was working on the # 6 belt, tightening a bolt by hand. The ratchet slipped and came back and hit the employee above the left eye. He sustained a small cut and lump above the left eye. Immediately after injury he was transported by a company official to the company doctor, who stitched him and returned him to work the same day.

SHOULDER PAIN, CHEST PAIN ON LEFT SIDE, DIFFICULTY BREATHING. GOT HURT OFF THE JOB, PULLED A CHEST MUSCLE, NOT WORK RELATED.

EMPLOYEE WAS GRINDING ON RED SCREEN WHEN GRINDER SUDDENLY SHUT OFF. EE SAW THAT IT HAD UNPLUGGED FROM THE ELECTRIC SOURCE, SO HE REACHED OVER AND PLUGGED IT BACK IN. THE SWITCH ON THE GRINDER WAS STILL ON THE ON POSITION, SO WHEN HE PLUGGEDIY IN, IT TURNED BACK ON AND IT MADE A SMALL CUTON HIS LEFT LEG JUST NELOW THE KNEE.
