The plant operator was using a 3/4" air impact gun to tighten bolts on the swing dye of the jaw crusher when the impact gun bounced back and pinched the operator's hand between the tool and the wall of the crusher.

Miner was assisting in replacing a screen on the bottom deck of the base plant. Miner raised his head in a confined environment and hit his head on a bolt that was on the strap of a guard over a cross tube.
