The vendor was delivering flashing to us and when he was returning to his truck a dog had bitten him.

He was using a 4"" hand grinder. The 4"" hand grinder got caught on a bar and it kicked out of his hands.

Absorption of radiations, caustics, toxic and noxious substances

Crusher with inadequate dust control was moved next to control shack, subjecting operator to high levels of rock dust. This resulted in shortness of breath and sore chest/lungs. This condition (excessive dust) persists. I am seeking medical attention today. I have missed three days of work so far. Time/date is approximate as condition grew worse over several weeks.

Fall onto or against objects

Employee was climbing on an excavator at the start of his shift when he slipped and hit his head on the counterweight. He suffered a minor concussion and minor bruising to his hand back, and shoulder. He is expected back to work in three days.

We were tightening a nut by swinging a heavy weight against a large wrench. The weight went under the wrench and employee's fingers were pinched between an ear on the weight and the wrench.
