Crusher with inadequate dust control was moved next to control shack, subjecting operator to high levels of rock dust. This resulted in shortness of breath and sore chest/lungs. This condition (excessive dust) persists. I am seeking medical attention today. I have missed three days of work so far. Time/date is approximate as condition grew worse over several weeks.

Employee was climbing on an excavator at the start of his shift when he slipped and hit his head on the counterweight. He suffered a minor concussion and minor bruising to his hand back, and shoulder. He is expected back to work in three days.
