Struck by falling object

The victim was operating a skid steer loader underneath a belt conveyor that was being dismantled. A co-worker was in an elevated manlift & was using a cutting torch to remove a piece of 4"" metal tubing attached to the leg supports of the belt conveyor. The tubing fell to the ground, tipped over into the front of the loader & struck the victim. He was hospitalized & died on 1/9/09.

Injury was using a 200 Lbs battering hammer which was suspended in air by the crane. while trying to loosen the large nut for the mantle liner on the short head cone crusher. his hand was pinched inbetween the front of the crane and the battering ram hammer. This caused a cut to his right hand.

EMPLOYEE HAD HIS FOOT IN BETWEEN THE GUARDRAIL AND THE JAW CRUSHER WHEN A ROCK SHOT UP AND HIT HIS FOOT - FRACTURED HIS TOE.

WHILE CHANGING V-BELT ON #1 SCREEN DECK, EMPLOYEE'S RIGHT FOREARM WAS CAUGHT BETWEEN GUARD AND THE BELT, CAUSING A LACERATION.
