Struck by flying object

Employee was replacing blow bars on the crusher. While removing the old blow bars with a hammer a piece of metal flaked off striking employee in the face.

Caught in, under or between a moving and a stationary object

While drilling, the Kelly Bar was drilled down to full depth. EE climbed on the drill deck to unlock the inner steel. As EE inserted the pry bar to disengage the J-Lock, the drill operator inadvertently put the drill steel in reverse. EE's arm was pinned between the pry bar and the mast of the drill. The injury resulted in swelling and a bruise to EE's right forearm.

Fall from machine

EMPLOYEE FELL APPROX. 4 FEET TO THE GROUND WHILE CLIMBING DOWN OFF OF THE 4600 DRAGLINE. HE RECEIVED A BROKEN WRIST ON HIS LEFT ARM AND A FRACTURE ON THE INSIDE OF THE PELVIS. EMPLOYEE DID NOT USE THE LADDER THAT WAS PROVIDED TO ACCESS AND EXIT THE EQUIPMENT.

Over-exertion in pulling or pushing objects

Employee stated he was pulling fuel hose from service truck to fuel a piece of equipment when he felt a pain in his lower back. He did not tell anyone about this until 9-8-05. He doesn't recall what day he actually did this or what time it was.

Struck against a moving object

Operator stated that he ran over a rock and that it jarred his back when the loader rocked from side to side. The rock was about 10 inches by 12 inches.
