Struck by falling object

Employee was feeding the chain, when a clay mass which had accumulated on the conveying equipment overhead dislodged and hit the employee on the back of the neck, causing bruising and swelling.

Struck by flying object

EE was crushing clay when a piece of grit got in his eye. He was unable to get grit out and his eye became swoole & red. He went to Dr. who said he had scratched his cornea. The Dr. bandaged the eye and advised the EE to take one day off work.

Struck by powered moving object

Track hoe operator was positioning track hoe to load clay trucks. Unknown to the operator, employee arrived at the pit and approached track hoe from the blind side. Hoe operator swung bucket around and struck employee in the upper back and shoulder knocking him to the ground.

Struck by flying object

EMPLOYEE WAS ON A PLATFORM PUNCHING OUT THE CLAY HOPPER WHICH HAD GOTTEN CLOGGED WITH WET CLAY. ANOTHER EMPLOYEE ON THE GROUND TOSSED UP A LUMP OF CLAY TO GET HIS ATTENTION. THE LUMP OF CLAY S TRUCK HIS LEFT EYE. HE RINSED HIS EYE WITH WATER AND COMPLETED HIS SHIFT. HE DID NOT TELL HIS SUPERVISOR. THE EYE BECAME MORE IRRITATED OVER THE WEEKEND. HE SAW A DOCTOR ON 9/15.

EE WAS CLEANING THE DRAG CHAIN WHEN HE SLIPPED, CAUSING HIS FOOT TO SLIDE UNDER THE CHAIN. THIS CAUSED A COMPRESSION INJURY TOHIS LEFT FOOT.
