THE GOUT CHAIN BEING USED HAD A TWIST IN IT. THE OPERATOR PICKED UP THE BLOCK BEFORE FLAGMAN COULD STOP HIM. THIS CAUSED THE CHAIN TO BREAK AND A SMALL PART OF THE CHAIN PUNCTURED THE LOWER AR M.
