EMPLOYEE BLED PRESSURE FROM TANKER. WENT TO UNHOOK HOSE. THERE WAS PRESSURE ON THE HOSE. HOSE BLEW OFF, HOSE CLAMPS CUT FINGERS. LACERATION TO LEFT INDEX FINGER REQUIRED 4 SUTURES.

DID NOT FOLLOW SAFE WORK PROCEDURES, GOT IN A HURRY. WAS ATTEMPTING TO REMOVE TIRE FROM 2 STAGE WHEEL, WHEN HE DID NOT RELEASE AIR PRESSURE/VALVE CORE. WHEN HE REMOVED LAST BOLT HOLDING IT TOG ETHER IT EXPLODED AND STRUCK HIM IN FACE.
