Fall to the walkway or working surface

EE was setting the electronic equipment to detect the centering of the holes to be drilled with the 3"" diameter drill. EE mistakenly leaned on a rod of the hole finder equipment and fell. EE, two days later on the phone, told me that EE had broken EE's wrist and ankle.

Contact with electrical current

The worker was working with our technician at the power cabin at the quarry. When our technician was not present in the cabin the worker did a non-authorized procedure in the power cabin and gets shocked. The skin on the worker's hands was burned.

Over-exertion (Not Elsewhere Classified)

2 weeks ago, employee informed me that EE reported a finger pain to the previous Quarry Manager in February. EE didn't know of a specific task or event that caused the pain, but felt repetitive work tasks may have been a factor. EE was taken to the hospital for treatment. This injury was never reported MSHA because I discovered this only 2 week ago.

Employee was breaking slabs with hammer and clever. A piece of steel came off the hammer and made a small puncture wound in his chest.

Two employee were carring a long piece of steel. When setting and be drillrod down it picked 1 finger as the rod flapped a the finger was between the steel rod and rock. This caused an injury to 1 finger that required stitches.

The victims ( previous owners) were on my property attemping to start a retired loader. The engine fired causing the loader to lunge in reverse running over the two victims.
