EMPLOYEE WAS PUSHING OPEN A GARAGE DOOR TO INSTALL SOME LIGHT AND FELT PAIN IN HIS RIGHT FOREARM. HE REPORTED IT TO COMPANY OFFICIAL BUT DID NOT GO TO DOCTOR. PAIN GOT WORSE AND EE WENT TO DOC TOR ON 3-31-01. DOCTOR PRESCRIBED MEDICATION ANDSPLINT TO HELP IMMOBILIZED ARM. RETURNED TO WORK.

WHILE REMOVING 7HE MIDDLE SECTION OF THE CRUSHER TO GAIN ACESS TO THE BOTTOM ROLLERS THE MIDDLE SECTION SLIPPED AND FELL ON THE LEFT HAND OF I NAME PINNING THE HAND AGAINST ANOTHER PART OF THE CRUSHER. DR SAI8 LIGAMENTS WERE TORN AND MINOR CUTS ON HAD REQUIRED 8 STITCHES
