EE re-assembled bearing in bearing housing. As afternoon progressed, EE's back stiffened up and began to hurt; EE worked till the end of day; went to the doctor the next day when back had not improved.

EE was clearing material buildup from a grizzly chute in advance of the primary crusher when a portion of the material dislodged and rolled to a point where it impacted his left ankle against a structural beam. This resulted in a fracture of the fibula at the ankle.

While tightening hood closure lock bolts on an impact crusher, using a slugging wrench and sledge hammer, employee struck an errant blow causing his right index finger to pass between the hammer handle and wrench. This resulted in the cutting and spreading of finger tissue beyond the knuckle. He was treated at emergency room where stitches were installed. He returned to work.

While cutting brush in a storage area chainsaw kicked from cut and contacted left knee causing a 1 1/2" cut. After cleaning and dressing at office it was decided to seek medical treatment. EE was taken to hospital where stitches were used to close the cut.

During start-up of inspection, stepped onto cab entry step (platform). After checking dump body for stuck material, slipped on frosty surface and fell 3' to ground landing on left shoulder. Suffered separated shoulder and has been off duty since.

EE suffered a seizure while in office prior to start of shift. Taken to Hospial by ambulance, release that afternoon. Referred to Neurologist, cause undetermined, possible side effect of medication, now discontinued. Released to return to work.

WHILE SHOVELING CRUSHED STONE FROM A CONVEYOR BELT, WHICH HAD STALLED AND BECOME OVERLOADED, IN PREPARATION FOR RESTARTING THE CONVEYOR, EE STATES THAT HE TURNED TO DISPOSE OF A SHOVEFUL OF ST ONE AND FELT A PAIN INHIS BACK WHICH SUBSIDED. STIFFNESS AND PAIN SET IN THAT EVENING. HESOUGHT MEDICAL ATTENTION THE FOLLOWING DAY AND WAS OFF WORK A TOTAL OF 3 DAYS.
