Over-exertion in lifting objects
The employee is an end dump driver. He got out of his end dump to pick up a rock that had fallen off his truck onto the haul road. When he picked it up he felt a pull or twist in his foot.

The loader operator was stepping down off of the 330 loader when he hit his left elbow on the bar.

Employee finished servicing truck and went to park it and fell from step approximately 4 foot to the ground on his side.

Struck against a moving object
Pit truck driver went to get out of the truck and his back felt stiff. The next morning he called in because he could not get out of bed. He went to the dr. and was told he had a pinched nerve in his lower back and would be off work the rest of the week.
