Employee was being lifted up on a forklift and put their hand between the plates. When coming down, the plates came together smashing three of EE's fingers.

While helping another miner repair a V belt on the sand screw a pipe wrench slipped out of the hand of the other miner striking EE in the forehead.

Caught in, under or between a moving and a stationary object

Welding on a new chute, EE asked for the assistance of a co-worker in turning over the chute. Co-worker lifted one side of the chute using a forklift, and as the chute turned over it bounced off the table and into EE forcing him into a metal table behind him.
