WHILE OPERATING MOTORGRADER, GRADING THE HAULAGEROAD, THE GRADER BLADE ABRUPTLY SNAGGED A LARGE BOULDER OBSTRUCTED IN THE HAULROAD SUBSURFACE CAUSING THE MOTORGRADER'S BLADE CONTROL LEVER TO K ICK BACK AND TWIST THE OPERATOR'S LEFT WRIST ANDCAUSE THE MOTORGRADER TO HALT.

A-PPROX 5-7 HOLES MISFIRED UPON DETONATING BLAST. AFTER CHECKING ALL DOWNHOLE LINES & SURFACE DELAYS, SHOT WAS CLEARED. ABOUT 40 MINUTES AFTER BLAST WAS DETONATED, THE MISFIRED HOLES IGNITED. THE CAUSE IS BELIEVED TO BE FROM PRIMERS FAILING TO DETONATE WHEN IGNITED. THE CAUSE IS STILL UNDER INVESTIGATION.
