EMPLOYEE WAS USING A LONG PRY BAR TO LOOSEN A HYDRAULIC CYLINDER HE WAS INSTALLING THAT HAD BECOME LODGED. AS THE CYLINDER CAME LOOSE IT SWUNG AROUND FORCING THE PRY BAR INTO EMPLOYEE'S LEG AB OVE KNEE.
EMPLOYEE WAS INSTALLING A BUSHING ON HYDRAULIC SHOVEL. HE WAS USING A HAMMER TO BEAT THE BUSHING INTO PLACE. A SMALL PIECE OF METAL BROKE OFF AND IMBEDDED INTO HIS LEG.
