Struck against stationary object

At around 4:45pm eastern time EE was exiting the WA700. As EE was coming down the ladder, EE got right hand caught on a cotter pin that was bent over at the top of the handrail and cut right hand needing 3 stitches.

A miner sustained a laceration and nondisp fx to the tip of the right long finger. The injured was preparing to replace a hydraulic hose on the WA 700 Front End Loader. While opening an access door EE states EE lost balance allowing the door to open fully catching the tip of the right long finger in a nip point between the cover EE was holding and another cover.

The injured strained his right shoulder when he slipped on the ladder for the primary crusher. Medical treatment did not occur until August 1, 2005.

The injured was helping a customer cartage truck driver open a stuck tailgate when the gate broke free crushing his left index finger. the injury consisted of a fractured finger.

THE EMPLOYEE WAS HELPING MOVE A TABLE IN THE O.C. LAB ON FEBRUARY 23, 2004, WHEN THE EMPLOYEE TWISTED HIS KNEE. HE WAS EXAMINED AT A HEALTH CLINIC AND WAS RELEASED BACK TO WORK. ON MARCH 16TH THE EMPLOYEE'S KNEE WAS STILL CAUSING HIM PAIN AND AN MRI REVEALED TORN CARTILAGE. THIS INJURY BECAME RECORDABLE ON MARCH 16, 2004 WHEN THE MRI REVEALED TORN CARTILAGE.
