Miner climbed over the top of pallets to verify staged pallets. After verification EE jumped from the top of the pallet to the ground and broke leg.

Miner was kneeling on gravel screen making repairs. When he stood up something tore in his knee.
Very cold weather with ice. Miner was loading super sacks with gravel and had his finger pinched by strap and forklift fork causing his finger to be lacerated and broken.

Sand plant operator was using a power drill and the drill bit locked up and twisted into his arm. The bone on the outside of forearm was broken.

Employee accessed an unauthorized area on a screen to free a frozen belt while the conveyor was running. His coat became entangled between the conveyor and belt pinning him against the two. The breaker popped due to the resistance. Employee was taken to hospital where x-rays indicated no broken bones or sutures required. Bruises to his upper left arm.

EE came to work complaining of a stiff back. Started working palletizing. Back began to hurt. relieved of duties and informed to rest. Decided to wash down gravel hopper without permission, Slipped and fell on pile of sand, did not re-injure back. Was advised to go home. Instead went to MD who prescribed bed rest for 2 days and return to work. employee has failed to return.

Employee worked normal work day. Went home and complained about lower stomach pain about 10 pm. Went to hospital ER where it was discovered he had an inguinal hernia. Surgery was scheduled for 6/22/06.

EMPLOYEE WAS TIGHTENING A BOLT ON A WATER PUMP ON THE MAIN DREDGE USING A 18"" WRENCH AND OPEN-END STEEL PIPE. THE PIPE SLIPPED OFF THE WRENCH, STRIKING HIM ABOVE THE LEFT EYEBROW, CAUSING A CUT THAT REQUIRED THREE SUTURES.
EE WAS ADJUSTING A CONVEYOR BELT ON AN ELEVATED SAND SCREEN (VIBRATING) USING A 17"" BOXED-END WRENCH. THE WRENCH CAME OFF THE BOLT AND HIS FORWARD INERTIA CAUSED HIM TO LOSE HIS BALANCE, ALLOWING HIS LEFT ARM TO SLIDE UNDER THE BELT AND GO AROUND THE END PULLEY. THERE APPEARS TO BE A COMPOUND FRACTURE OF THE L. FOREARM PLUS ABRASIONS FROM THE L. SHOULDER TO THE L. HAND.

EE WAS WORKING ON A PIECE OF EQUIPMENT THAT HAD BROKEN DOWN IN THE BAGGING SHED. HE HAD REMOVED PARTS FROM THIS EQUIPMENT AND WAS PUTTING IT IN THE BACK OF A PICKUP TRUCK. HE WAS STANDING BEHI ND THE PICKUP WHICH WAS PARKED DIRECTLY IN FRONT OF THE BAGGING SHED DOORWAY, WHEN HE WAS STRUCK BY THE FORK TRUCK WHICH WAS BACKING UP.

EE HAS BEEN BOTHERED BY CARPAL TUNNEL IN BOTH WRISTS FOR A NUMBER OF YEARS AND HAS DECIDED TO HAVE ELECTIVE SURGERY ON BOTH WRISTS THIS DAY.
EE WAS WALKING UPHILL IN A COVERING OF SNOW. SHE SLIPPED AND FELL BREAKING HER LEFT WRIST.
