Employee was clearing catwalk when a rock fell and hit EE in the ribs.

Employees were working on top of surge tunnel setting form work in place to add decking for a concrete pour. The main beam on the form work broke causing the form work to collapse. Both employees fell and one employee's retractable lanyard prevented the employee from striking the ground. The second employee struck the wall in a swinging affect or hit the ground.

Employee stepped on a rock and twisted their foot, which resulted in a broken foot.

EE was unreeling a water hose as EE walked up conveyor #2. When EE attempted to turn on the water, EE realized that there was pain in EE's arm. Update: Upon further review, right rotator cuff is torn.

EE was preparing pump to be removed from pit. EE began disconnecting the flange connection on the discharge side of the pump. The pressure on the line shot water upwards out of the flange. The EE alleges that EE's hard hat and safety glasses came off as a result of the water pressure, and the white of EE's left eye received a small laceration.

While unloading drill pipe from the drill truck, the miner was lowering the drill pipe to the ground using a chain. While lowering, the drill steel slipped within the chain causing EE's right little finger to get caught between the steel being lowered and another piece of drill steel on the ground.

While welding, slag fell on top of EE's steel toed boot and burned through the top of the shoe. The EE was taken to the clinic and diagnosed with three small first degree burns. The wounds were cleaned thoroughly, and EE was given instructions for care and released with no restrictions or follow-up. Became reportable on 9/14/2018 after an infection was diagnosed.

Employee injured EE's right arm while using a 4lb sledge hammer to track a conveyor belt.

The employee was climbing the stairs on the way to Tower #2 to change screens. When EE reached the platform, EE started to turn to the side and put EE's hand on the handrail when EE felt a "pop" in EE's lower back. EE was not carrying anything while climbing the stairs.

The injured EE and coworkers were replacing the motor on a vertical pump. The pump was locked out and a backflow preventer was between the pump and the plant. Using a wrench to adjust the impeller, it began to spin jerking the wrench from coworker's hand & striking the injured EE's wrist. The backflow preventer leaked back and back stop failed allowing it to rotate backwards.

Crew was turning the 7ft short head crusher bowl in order to remove it for repairs. Injured EE was pushing on a pry-bar to rotate the crusher bowl. The bar slipped out of the cylinder hole in the bowl and the employee heard a "pop" and immediately felt pain in the left elbow. The employee was taken to the emergency room for evaluation.

Injured EE unbuckled the discharge line on a pit pump. As ee released the QD coupling water struck ee in the face causing ee to jump backwards and trip and slide down the embankment to the sump. EE struck several rocks as ee traveled down the slope and this caused injury to face and shoulder. EE walked up the ramp to where a coworker was on the forklift moving material.

The rod that holds an access door open failed and the door was not otherwise secured. The door fell and struck a technician in the head. In turn EE's head hit struck the machine and it cut EE's forehead and knocked out a tooth.

Employee was diagnosed with a bilateral sensorineural hearing loss.

EE was lifting a wheel flange with another teammate and felt a sharp pain in his back while lifting the flange, then he finished the job and then filed the work injury.

EE was removing a drive pulley guard when it shifted and pinched his finger between the guard and the support leg causing a laceration. EE received several sutures.

Employee reached down to pick up a chain. When he stood up he was a little dizzy and fell backwards. He stuck his left arm out to break the fall and dislocated his elbow.

EE was changing our wear shroud on a CAT 990 loader bucket. The bolt dropped past the shank allowing the bolt to spin. The EE used his gloved hand to push the bolt back up and inadvertently hit the trigger to the impact wrench. This caused the bolt to spin, cutting through the glove and cutting the EE's hand.

EE was diagnosed with bilateral sensorineural hearing loss.

Employee was diagnosed with a noise induced hearing loss after a STS was detected.

Employee took voluntary audiometric test that indicated a possible threshold shift. An appointment was made with a Dr of Audiology for 7/7/2011. The employee was tested and a STS was confirmed. The report was sent to the company in which the Dr stated that the "Testing does not provide obvious evidence that the STS is entirely due to work environment"

EE had an odd feeling in his face and asked another EE if he looked ok. The coworker saw his face was drooping and took the EE to the foreman who feared he could be a stroke and called for an ambulance. Paramedic evaluated the EE and transported him to the hospital. The EE was diagnosed with Bells Palsy, a nerve condition and released from the hospital.

EE called his plant manager and complained of chest pains. The manager called for an ambulance and EE was transported to the emergency room. EE has been diagnosed with arterial blockage. This event is not work related. As of 8/18/2010 the employee has been release by his doctor to return to work.

Driller cutting blasthole tubes with knife. Blade released and cut drillers hand.

EE was cutting a wire tie from a bundle of computer wires. The knife blade broke causing EE's hand to strike a piece of metal lacerating the tip of his thumb & nail. The injury was treated with 1st aid & the bleeding stopped. EE returned to work requesting no further medical treatment. He self treated for 5 days but suffered periodic bleeding. On 2/6/09 he was sent to dr.

Employee closed van door on right index finger.

Employee burned himself installing Skirt Rubber. First Aid was administered on site and the injury only appeared to be a red spot on the inner arm. The burn became infected and the employee was sent to the doctor on 09/24/2007. He was prescribed antibiotics and returned to work that day with no restrictions.

While loading 50# bags of ANFO onto a shot, felt a sharp pain in lower back

Replacing pan liner on 6B feeder, cleaning a bolt, bolt hit with impact wrench by employee. Head of bolt cut finger.

Employee had finished his job for the night, as he walked down the steps from his work area he step on a rock and turned his ankle.

On September 12, the employee was cutting off old patches in a chute. He then was chipping the hot slag when a small piece struck his face below his safety glasses. The injury did not become recordable until 10/12/06 when he went to the doctor for the injury. His eye became infected as a result of the minor burn.

The employee was climbing the ladder of a piece of mobile equipment and strained his back. He did not seek medical treatment until 09/06/06.

The employee was pulling slack out of wire, in order to use a boom truck to move the wire and strained his back. The employee did not seek immediate medical treatment. He went to the doctor on July 10th.

Employee was loading a 50-ton battery.

Driving off road truck, going down ramp in pit to dump his load and said brakes failed to stop the truck. Operator ran into pile of rocks to avoid hitting another truck. Supv. said when he got to the accident to check on operator, he noticed the brake pedal was in front of driver seat and had come loose from mount.

Employee was changing Canica Impeller, strained shoulder while placing hammer in machine. The employee did not report the injury to management and seek medical treatment until 11/15/2005.

The employee was using an air compressor to grease the plant. The air compressor ran out of gas. While refueling the compressor some gas got onto the exhaust manifold and flamed up, causing burns to his hand and forearm.

Employee was moving new hammers for a Canica crusher in a box. Employee dropped hammer, striking left hand. Employee suffered contusion to left middle finger.

Employee is the pit foreman and was watching another employee remove a bushing by hitting it with a hammer. Once the bushing was hit, a piece broke off and struck the employee in the thigh by the groin. The injury resulted in stitches.

The canica crusher plugged up & employee climbed on top of the crusher to find the problem. He slipped on the material striking his knee againist the crusher lid. This resulted in a small laceration requiring no stitches, but he was on modified duty for two days.

Mechanic repaired line on backhoe. When he started to step off track onto the step, he slipped and turned ankle.

The employee was removing the pin for the bucket on a backhoe and was moving the pin to the back of a truck and felt a pain in his lower back. The employee did not report the injury until the end of the shift. The employee is on modified duty with a lifting restriction.

Employee was assisting with installation of grizzly bars in lower feeder when chain slipped, causing bars to pinch thumb between side of feeder and bars resulting in stitches to his left thumb and a broken knuckle. Employee is performing his normal duties, but did recieve a prescription.

Employee was washing out by crusher when a spider bit him in his right hand.

EMPLOYEE WAS CHECKING GREASE (BACKHOE) AND SLIPPED AND FELL, INJURING HIS NECK.

EE WAS REMOVING ROCK FROM STATIONERY BELT WHEN ROCK CAME DOWN THE BELT, STRIKING EE'S HAND. EE SUFFERED A FRACTURE INJURY TO RIGHT HAND.

EE WAS CARRYING A PAN LINER FOR BOTTOM OF FEEDER INTO 9'BLENDING TUNNEL TO REPAIR FEEDER AND INJURED ANKLE.

EMPLOYEE WAS SPLITTING A SAMPLE, DUMPING IT INTO SCREEN AND STRAINED BACK.

BACKHOE WAS LOADING OFF ROAD TRUCK WHEN A BOULDER FELL FROM BLASTED MATERIAL ON HIGH WALL & HIT BACK OF THE OFF ROAD TRUCK WHICH EE WAS DRIVING.THE JOLT THREW HIM AROUND.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE WELD A PIPE. THE EMPLOYEEN DID NOT WEAR HIS WELDING HOOD THE ENTIRE TIME WHILE ATTEMPTING TO WELD THE PIPE.

A DRILL ROD CAME LOOSE FROM THE ROD LOCK AT THE TOP OF THE CAROUSEL. THE DRILLER WHO WAS INJUREDCLIMBED UP THE TOWER TO PUSH THE DRILL ROD INTO POSITION. HIS HELPER LOWERED THE TOP HEAD ONTO THE DRILL ROD TO PUT IT INTO THE CAROUSEL. WHEN LOWERING, THE DRILLER ON THE TOWER HAD HIS HAND IN THE PATH OF A TRAVELING BLOCK & HIS HAND WAS CAUGHT BETWEEN THE BLOCK & THE TOWER.

EE WAS WELDING IN THE JAW FEEDER. HE WAS UNAWARETHAT HIS WELDING HOOD WAS FAULTY, THUS CAUSING HIS EYE INJURY.

WAS WORKING ON DUST SUPPRESSION SYSTEM FOR THE PRIMARY CRUSHER FROM A PORTABLE LADDER. EE LOST HIS FOOTING ON THE PORTABLE LADDER HE WAS WORKING ON AND FELL CAUSING HIM TO FRACTURE HIS RIBS.

EMPLOYEE WAS ATTEMPTING TO CLEAN A SCREEN THAT HAD BLINDED OVER WITH A CHIPPING HAMMER. WHILE DOING SO THE EMPLOYEE SLID HIS SAFETY GLASSES TO THE LOWER BRIM OF HIS NOSE, CONTINUING TO CLEAR THE SCREEN THE HAMMER THE EMPLOYEE WAS USING STRUCK SOME MATERIAL THAT FLEW UP AND STRUCK THE EMPLOYEE IN THE EYE.

EE WAS ATTEMPTING TO TEAR DOWN A CONE CRUSHER. WHILE PULLING THE CRUSHER BOWL WITH A CABLE, THE CABLE GOT STRUCK. THE EE THEN ATTEMPTED TO FREE THE CABLE WHEN HIS HAND GOT CAUGHT UP BETWEEN TH E CABLE AND THE CRUSHER. EE DID RECEIVED STITCH ES IN HIS FINGER.

EMPLOYEE WAS REPAIRING A HYDRAULIC LINE ON A PIECE OF HEAVY MACHINERY. HE USED A BOX KNIFE RAZOR BLADE ON A PIECE OF RUBBER HOSE, SLIPPED, AND CUT FINGER.

EE WAS CUTTING A PIECE OF RUBBER WHEN THE RAZOR KNIFE HE WAS USING SLIPPED & CUT HIS KNEE

EE WAS HOLDING A WRENCH ON A HOSE FITTING WHILE EE LOOSENED IT. THE WRENCH BROKE FREE & EE LOST HIS BALANCE. WHILE TRYING TO REGAIN HIS BALANCE, HE RAISED HIS ARMS & STRUCK COWORKER IN THE LIP .

USING PRY BAR TO LOOSEN BRIDGED MATERIAL. ROCK STRUCK RIGHT HAND AND CUT.

REPAIRING CONVEYOR BELT. BELT CLAMP BROKE STRIKING ON RIGHT THIGH, JUST ABOVE KNEE.

EMPLOYEE HAD FINISHED FUELING TRUCK. GOING UP LADDER, SLIPPED AND CAUGHT HIMSELF WITH RIGHT ARM INJURING HIS RIGHT SHOULER.

EE WAS DRIVING A PIN OUT OF A DIFFERENTIAL WITH A 6 FT PIPE. WHEN PIN CAME OUT, DIFFERENTIAL FELL ON LOWER PART OF RT ARM. THIS BROKE WRIST IN 2 PLACES.

EE WAS IN AREA WHERE TWO OTHER EE WERE USING A SLEDGE HAMMER TO REMOVE A BUSHIND FROM A SHAFT A FRABMENT BROKE OFF THE BUSHING AND TRAVELLED 10-15 FEET STRIKING EE IN THE MOUTH. REQUIRING STIT CHES AND LOSS OF A TOOTH.

WIND BLOWN DEBRIS TO RIGHT EYE. WAS WEARING SAFETY GLASSES.

BENDING OVER TO FILL OIL TANK. BOLT FELL FROM OVRHEAD WORK STRIKING ON BASE OF NECK.

EE WAS WALKING ON GRAVEL PARKING LOT FROM HIS VEHICLE TO THE WAREHOUSE OFFICE AND SLIPPED TWISTING HIS KNEE.

EE WAS GRINDING PIECE OF WEAR METAL. SMALL FRAGMENT WENT UNDERSAFETY GLASSES AND ENTERED RIGHT EYE.

EMP WAS FILLING BRAKE CHAMBER WITH HYDRAULIC OILUSING A 1 GALLON CONTAINER AND TWISTED BACK.

EE TIGHTENING BOLTS WITH WRENCH AND STRAINED RIGHT WRIST.

EMPLOYEE WAS POURING HEATED ROCK INTO A WATER BATH TO PERFORM A SPECIFIC GRAVITY TEST. THE ROCK CAUSED THE WATER TO CREATE STEAM AND IT BURNED THE EMPLOYEE'S THUMB.

EMPLOYEE WAS CHECKING THE GRIZZLY AT THE SAND PLANT & THE WIND BLEW SAND INTO HIS EYE

EMPLOYEE WAS USING A CHEMICAL TO CLENA OUT A FROZEN AIR LINE. THE WIND WAS BLOWINGAND HIS HARD HAT AND GLASSES BEGAN TO FALL OFF JUST AT THE AIR LINE THAWED. THE CHEMICAL BLEW INTO HIS NOW E XPOSED EYES.

EMPLOYEE WAS LEAVING THE ELECTRICAL POWER ROOM TO THE MILL HE STEPPED ON A 2IN X15IN BOLT LAYING IN THE WALKWAY HE STRAINED HIS ANKLE

EMPLOYEE WAS USING A STEAM CLEANER TO CLEAN EQUPMENT. THE HANDLE SLIPPED OUT OF HIS HAND AND SPRAYED HIS ANKLE WITH HOT WATER AND STEAM.

EMPLOYEE WAS RIDING IN A PICKUP TRUCK & A PIECE OF FOREIGN MATERIAL BECAME EMBEDDED IN HIS EYE IT MAY HAVE BLOWN OUT OF THE AIR CONDITIONER VENT

EMPLOYEE WAS WALKING NEXT TO A HYDRAULIC ROCK BREAKER CRAWLER TRACK MACHINE A PIECE OF METAL BROKE OFF THE MACHINES TRACK & HIT HIM IN THE ARM IT CAUSED A PUNCTURE WOUND & THE PIECE OF METAL W AS REMOVED BY THE DOC

EMPLOYEE WAS ATTEMPTING TO BREAK AN OVERSIZED ROCK THAT WAS HUNG UP IN THE PRIMARY CRUSHER. THE EMPLOYEE STRUCK THE ROCK WITH A SLEDGE HAMMER AND A PIECE BROKE OFF AND STRUCK THE EMPLOYEE.

EMPLOYEE WAS CLOSING DOOR TO LOADER AND CAUGHT TWO FINGERS IN THE DOOR.

TWO ELECTRICIANS WERE INSTALLING A LIVE TRANSMITTION FEED LINE THE MAIN DISCONNECT FOR A JAW CRUSHER. THE LINES (480 VOLT) HAD BEEN INCERTED INTO THE LUGS. ONE EMPLOYEE TURNED TO GET A WRENCH & THE OTHER EMPLOYEE TRIED TO REINSERT ONE LEG FURTHER INTO THE LUG. IT CAME OUT OF THE LUG & CONTACTED THE EDJOINING LUG CAUSING IT TO ARC.

TWO ELECTRICIANS WERE INSTALLING A LIVE TRANSMITTION LINE INTO MAIN DISCONNECT FOR A JAW CRUSHER. THE LINES (480 VOLT) HAD BEEN INCERTED INTO THE LUGS. ONE EMPLOYEE TURNED TO GET A WRENCH & TH E OTHER ELECTRICIAN TRIED TO RE-INSERT ONE LEG FURTHER INTO THE LUG. IT CAME OUT OF THE LUG & CONTACTED ONE OF THE OTHER LUGS CAUSING AN ELECTRICAL ARC.

EMPLOYEE WAS REPLACING A 110V CONTROL RELAY. WHEN THE REPLACEMENT WAS PLUGGED IN, IT ARCED AND BURNED THE MANS HAND. THE POWER HAD NOT BEEN DISCONNECTED BEFORE REPLACING THE DEFECTIVE RELAY.

EMPLOYEE WAS USING A WEDGE AT THE JAW CRUSHER THE ROCK BREAKER WAS OPERATIONAL BUT WOULD NOT REACH THIS WEDGED ROCK.ANOTHER ROCK FELL ON THE WEDGE AND FORCED IT DOWN.THE EMPLOYEE HELD ONTO THE WEDGE AND THIS CAUSED A LUMBAR STRAIN.

EMPL WAS CHANGING DRIV E SHAFT ON HAULAGE TRUCK. WRENCH SLIPPED & EMPL CUT HAND ON BURR ON DRIVE SHAFT.

EMPLOYEE WAS REMOVING A NUT HOLDING A HYDRAULIC LINE IN PLACE. HE BEGAN TURNING THE NUT WITH HIS LEFT HAND AND CUT IT ON A BURR. THE HAND BECAME INFECTED 11 DAYS LATER AND HE SPENT 2 DAYS IN T HE HOSPITAL.

EMPLOYEE STEPPED OUT OF AN ELECTRICAL SUPPLY TRAILER AND STEPPED ONTO A PIECE OF PIPE THAT MOVED. HE SPRAINED HIS ANKLE.

EE WAS HELPING UNLOAD D-8 DOZER FROM LOW BOY. HE WAS LOOSENING BUCKED CHAINS. WHEN HE LOOSENED BUCK IT THREW UP PIPE AND PIPE HIT HIM IN THE CHIN. HE THOUGHT HIS MOUTH WAS SORE FROM THE IMPACT OF THE PIPE, BUT UPON EXAMINATION BY DENTIST DISCOVERED BROKEN TEETH.

EMPL WAS MOVING SOME CONCRETE FORMS. HE STRAINED SOME MUSCLES IN BACK & HAD PHYSICAL THERAPY TO HELP HIM RECOVER.

EMPLOYEE WAS HELPING ANOTHER PERSON MOVE 55 GALLON DRUMS OF ANTIFREEZE TO A LIFT AT THE REAR OF A TRUCK. 5 DAYS LATER HE COMPLAINED OF BACK PAIN. EMPLOYEE MISSED WORK DAYS.

EMPLOYEE WAS CLIMBING DOWN FROM HIS HAULAGE TRUCK AND SLIPPED. HE FELL TO THE GROUND AND SPRAINED HIS ANKLE.

EMPL WAS MOVING FEED PLATE TO CONE CRUSHER. HE DROPPED PLATE & IT STRUCK HIM IN LEG. HE WAS GIVEN PRESCRIBED MEDICATION.

EMPL WAS CLIMBING DOWN OUT OF HAULAGE TRUCK & STEPPED ON LOOSE ROCK. HE SPRAINED ANKLE. HE WAS GIVEN PRESCRIPTION MEDICATION.

EMPLOYEE BURNED HIS EYES WORKING CLOSE TO PEOPLE WELDING IN A FEEDER. HE WAS WEARING DARK SAFETY GLASSES BUT THEY WEREN'T ADEQUATE PROTECTION.

EMPL PICKED UP TIRE TO INSTALL IT ON GREASE TRUCK STRAINED SOME BACK MUSCLES.

EMPLOYEE WAS USING A HAMMER TO DRIVE THE BEARING RACE OUT OF THE HOUSING OF A JAW CRUSHER PITMAN A PIECE OF METAL BROKE OFF THE RACE & STRUCK THE MAN IN H IS RT.THIGH

EMPLOYEE SLIPPED GETTING OUT OF A HAUL TRUCK AND STRUCK HIS KNEE ON THE STEPS. HE HAD 6 S STITCHES APPLIED TO THE WOUND.

EMPLOYEE WAS CARRYING BOXED DOWN STAIRS AND SLIPPED AND FELL. HE STRAINED SOME MUSCLES IN HIS BACK AND WAS GIVEN MEDICATION.

EMPLOYEE WAS UNDERNEATH A DROP BALL WORKING ON THE UNDER CARRIAGE. HE GOT SOME GRIT IN HIS EYE. HE WORKED THE REST OF THE DAY AND HIS EYE SWELLED OVERNIGHT. HE WAS TREATED BY THE DOCTOR WITH S OME PRESCRIPTION MEDICATION.

EMPLOYEE WAS PULLING ON A PICE OF WIRE ROPE AND STRAINED A MISCLE IN HIS RIGHT SHOULDER.

EMPL WAS DRIVING EUC TRUCK & HIT BUMP IN ROAD JERKED HIS BACK & EXPERIENCED SEVERE PAIN.

EMPLOYEE WAS DRIVING A U-JOINT OUT OF A HAULAGE TRUCK,APIECE OF METAL CHIPPED OFF THE U-JOINT AND STRUCK THE EMPLOYEE IN THE LEFT FOREARM.MEDICATION WAS GIVEN.

EMPLOYEE SPRAINED HIS ANKLE WHILE CLIMBING OUT OF A HAULAGE TRUCK.

EMPLOYEE WAS TI1HTENING CONE WEDGES BY HITTING THEM WITH A SLEDGE HAMMER HE STRUCK A WEDGE AND THE HAMMER GLANCED OFF IT AND MASHED HIS FINGER BETWEEN THE HANDLE OF THE HAMMER AND A HANDRAIL

EMPLOYEE WAS SE9VICING HIS HAUL TRUCK (GREASING). HE STOOD UP & BUMPED HIS HEAD ON THE FRAME.

EMP WAS LOOSENI3G A BOLT ON AN ELECTRIC MOTOR. THE WRENCH HE WAS USING BROKE AND CAUSED THE EMP TO LOSE HIS BALANCE AND FALL 5 FT TO THE GROUND

EMP WAS INJURED3WHEN WIND BLEW A LOADER SHUT AGAINST HIS RING FINGER ON RT HAND

EMPLOYEE WAS CH5INING PIPE TO A FRONT END LOADER.THIS WAS 8" STEEL PIPE.A PIECE ROLLED SUDDENLY AND STRUCK THE EMPLOYEE'S FOOT.HIS BIG TOE AND LITTLE TOE WERE BROKEN.

EMPL DESCENDING GRADE INTO STOCKPILE AREA.HIS BRAKES FAILED AND HE TURNED INTO STOCKPILE TO STOP IT.UPON EXAM OF VEHICLE,LEAKINGBRAKE LINE WAS FOUND.TRUCK WAS PUT OUT OF GEAR AND EMERGENCY BRA KE WASNT ENGAGED.IT WAS IN WORKING ORDER.EMPL DIDNT GEAR VEHICLE DOWN.

EMPLOYEE WAS DIGGING ROCK OUT OF THE JAW CRUSHER. SOME LOOSE ROCK IN THE FEEDER SLIPPED & FELL ON THE EMPLOYEE\S FOOT. IT TORE SOME LIGAMENTS & THE EMPLOYEE MISSED 3 DAYS WORK.

WIND BLEW DOOR TO SCALEHOUSE CLOSED. EMPLOYEE TRIED TO CATCH DOOR AND DOOR CAUGHT HIS WRIST BETWEN FRAME AND DOOR.

EMP WAS TAKING THE INNER RACE FROM A BEARING OFF OF THE PITMAN SHAFT FROM A JAW CRUSHER WORKER HIT INNER RACE WITH A SLEDGE HAMMER ATTEMPTING TO DISLODGE RACE FROM SHAFT

EMPLOYEE WAS TRYING TO BREAK FREE A SCREEN RACK ON A SHAKER. RACK AWAS BEIANG PULLED USING CABLES TIED TO A LOADER, RACK GOT HUNG UP, EMPLOYEE GOT UP ON SHAKER USING A SLEDGE HAMMER TO BREAK ROCK FREE. ROCK SPRUNG BACK WHEN EMPLOYEE STRUCK IT HITTING EMPLOYEE ON THE LEG.
