Employees were working on top of surge tunnel setting form work in place to add decking for a concrete pour. The main beam on the form work broke causing the form work to collapse. Both employees fell and one employee's retractable lanyard prevented the employee from striking the ground. The second employee struck the wall in a swinging affect or hit the ground.

EE was unreeling a water hose as EE walked up conveyor #2. When EE attempted to turn on the water, EE realized that there was pain in EE's arm. Update: Upon further review, right rotator cuff is torn.

EE was preparing pump to be removed from pit. EE began disconnecting the flange connection on the discharge side of the pump. The pressure on the line shot water upwards out of the flange. The EE alleges that EE's hard hat and safety glasses came off as a result of the water pressure, and the white of EE's left eye received a small laceration.

While unloading drill pipe from the drill truck, the miner was lowering the drill pipe to the ground using a chain. While lowering, the drill steel slipped within the chain causing EE's right little finger to get caught between the steel being lowered and another piece of drill steel on the ground.

While welding, slag fell on top of EE's steel toed boot and burned through the top of the shoe. The EE was taken to the clinic and diagnosed with three small first degree burns. The wounds were cleaned thoroughly, and EE was given instructions for care and released with no restrictions or follow-up. Became reportable on 9/14/2018 after an infection was diagnosed.

The employee was climbing the stairs on the way to Tower #2 to change screens. When EE reached the platform, EE started to turn to the side and put EE's hand on the handrail when EE felt a "pop" in EE's lower back. EE was not carrying anything while climbing the stairs.

The injured EE and coworkers were replacing the motor on a vertical pump. The pump was locked out and a backflow preventer was between the pump and the plant. Using a wrench to adjust the impeller, it began to spin jerking the wrench from coworker's hand & striking the injured EE's wrist. The backflow preventer leaked back and back stop failed allowing it to rotate backwards.

Crew was turning the 7ft short head crusher bowl in order to remove it for repairs. Injured EE was pushing on a pry-bar to rotate the crusher bowl. The bar slipped out of the cylinder hole in the bowl and the employee heard a "pop" and immediately felt pain in the left elbow. The employee was taken to the emergency room for evaluation.

Injured EE unbuckled the discharge line on a pit pump. As ee released the QD coupling water struck ee in the face causing ee to jump backwards and trip and slide down the embankment to the sump. EE struck several rocks as ee traveled down the slope and this caused injury to face and shoulder. EE walked up the ramp to where a coworker was on the forklift moving material.

The rod that holds an access door open failed and the door was not otherwise secured. The door fell and struck a technician in the head. In turn EE's head hit struck the machine and it cut EE's forehead and knocked out a tooth.

EE was changing our wear shroud on a CAT 990 loader bucket. The bolt dropped past the shank allowing the bolt to spin. The EE used his gloved hand to push the bolt back up and inadvertently hit the trigger to the impact wrench. This caused the bolt to spin, cutting through the glove and cutting the EE's hand.

Employee took voluntary audiometric test that indicated a possible threshold shift. An appointment was made with a Dr of Audiology for 7/7/2011. The employee was tested and a STS was confirmed. The report was sent to the company in which the Dr stated that the "Testing does not provide obvious evidence that the STS is entirely due to work environment"

EE had an odd feeling in his face and asked another EE if he looked ok. The coworker saw his face was drooping and took the EE to the foreman who feared he could be a stroke and called for an ambulance. Paramedic evaluated the EE and transported him to the hospital. The EE was diagnosed with Bells Palsy, a nerve condition and released from the hospital.

EE called his plant manager and complained of chest pains. The manager called for an ambulance and EE was transported to the emergency room. EE has been diagnosed with arterial blockage. This event is not work related. As of 8/18/2010 the employee has been release by his doctor to return to work.

EE was cutting a wire tie from a bundle of computer wires. The knife blade broke causing EE's hand to strike a piece of metal lacerating the tip of his thumb & nail. The injury was treated with 1st aid & the bleeding stopped. EE returned to work requesting no further medical treatment. He self treated for 5 days but suffered periodic bleeding. On 2/6/09 he was sent to dr.

Employee burned himself installing Skirt Rubber. First Aid was administered on site and the injury only appeared to be a red spot on the inner arm. The burn became infected and the employee was sent to the doctor on 09/24/2007. He was prescribed antibiotics and returned to work that day with no restrictions.

On September 12, the employee was cutting off old patches in a chute. He then was chipping the hot slag when a small piece struck his face below his safety glasses. The injury did not become recordable until 10/12/06 when he went to the doctor for the injury. His eye became infected as a result of the minor burn.

Driving off road truck, going down ramp in pit to dump his load and said brakes failed to stop the truck. Operator ran into pile of rocks to avoid hitting another truck. Supv. said when he got to the accident to check on operator, he noticed the brake pedal was in front of driver seat and had come loose from mount.

The employee was using an air compressor to grease the plant. The air compressor ran out of gas. While refueling the compressor some gas got onto the exhaust manifold and flamed up, causing burns to his hand and forearm.

Employee is the pit foreman and was watching another employee remove a bushing by hitting it with a hammer. Once the bushing was hit, a piece broke off and struck the employee in the thigh by the groin. The injury resulted in stitches.

The canica crusher plugged up & employee climbed on top of the crusher to find the problem. He slipped on the material striking his knee againist the crusher lid. This resulted in a small laceration requiring no stitches, but he was on modified duty for two days.
