ADDING 40' PIECE OF PIPE ON LAKE BEHIND DREDGE. THE WRENCH THE EE WAS USING SLIPPED ON BOLT BEING TIGHTENED AND CAUSED BOAT TO SHIFT AND EMPLOYEE FELL INTO FLOOR OF BOAT. EMPLOYEE CAUGHT HIMSE LF ON HIS HANDS IN FLOOR OF BOAT.

I NAME WAS LOOSENING A BOLT ON A SCREEN WITH A RATCHET. HE HIT HIMSELF IN HEAD WITH RATCHET WHEN IT SLIPPED ON BOLT. CUT HIM AT THE END OF HIS RIGHT EYEBROW BY THE TEMPLE. A DR PUT TWO STITCHE S IN IT.

I.NAME TOLD HIM TO DO SOME WELDING. I.NAME LEFT AND THE GOGGLES WERE IN HIS TRUCK. HE WELDED ANYWAY. HE CAME INTO OFFICE ABOUT 1:00 AND SAID HE HAD SOMETHING IN HIS EYE. I.NAME FLUSHED IT AND SENT HIM TO DOCTOR IN BEDFORD. HE CHECKED HIS EY E AND COULDN'T FIND ANYTHING IN IT BUT PUT A PATCH OVER IT.
THE INJURED STUCK THE HANDLE OF THE SHOVEL THROUGH THE GUARD INTO WHEEL OF CONVEYOR TO CLEAN MUD OF AROUND THE WHEEL AND THE WHEEL GRABBED THE SHOVEL HANDLE CAUSING THE TOP OF THE SHOVEL BLADE TO CRAM THE INJURED'S RIGHT HAND INTO GUARD ON CONVEYOR BELT.HE WAS NOT INSTRUCTED TO DO THIS.HE WAS JUST OVER-AMBITIOUS.
