EMPL HANGING TRAILING CABLES TO MINE ROOF.ON COMPLETING TASK REALIZED HE MUST SECURE EXCESS SLACK IN CABLES TO ROOF SO MOBILE EQUIP COULD PASS BENEATH.BEGAN PULLING ON SHUTTLE CAR TRAILING CA BLE SO HE WOULD HAVE ENOUGH SLACK TO SECURE CABLE TO NEARBY ROOF BOLT PLATE.SUDDENLY BEGAN EXPERIENCING PAIN IN LOWER REGION OF BACK.
RF FALL OCC IN NUMBER 2 ENTRY 60' INBY SPAD 9091. 20'W 20'L 15' THICK
SUBJECT WAS IN PROCESS OF INSTALLING PERMANENT ROOF SUPPORTS. AS SUBJECT WAS TIGHTENING ROOF BOLT PIECE OF COAL BECAME DISLODGED FROM ROOF STRIKING HIM IN LEFT EYE.
VICTIM WAS OPERATING THE LEFT HEAD OF A DUAL HEAD ROOF BOLTER. HE GRABBED THE WRONG CONTROL LEVER AND SWUNG THE HEAD OF THE BOLTER INTO HIMSELF. VICTIM STATED THAT HE EXPERIENCED A SUDDEN PA IN IN HIS BACK WHEN THE HEAD PUSHED HIM TOWARD THE LEFT RIB.

A ROOF FALL OCCURRED NUMBER SIX ENTRY APPROX 180FT INBY SPAD NUMBER 9179 FALL MEASURED APPROX 24FT WIDE 28FT LONG & 6FT THICK FALL OCCURRED IN AN INTERSECTION AREA OF THE FALL WAS A NEWLY CREA TED CUT & HAD NOT BEEN BOLTED FALL PARTIALLY COVERED A CONTINUOUS MINER
VICTIM WS AWARDED A LUMP SUM OF $13,776.88 (RETRAINING INCENTIVE BENEFITS) AS A RESULT OF BEING EXPOSED TO THE HAZARDS OF THE DISEASE OF COAL WORKER'S PNEUMOCONIOSIS. MARTIN COUNTY COAL CORP. , HAS BEEN ASSIGNED RESPONSIBILITY FOR PROVIDING THE BENEFITS. SUBJECTS IS CONTINUING TO WORK.

VICTIM WAS OPER5TING A 21SC SHUTTLE CAR HAULING COAL FROM A CONTINUOUS MINER. WHEN THE SHUTTLE CAR HAD BEEN LOADED THE VICTIM STEPPED OUTOF THE OPERATORS COMPARTMENT TO CHANGE SEATS AND FACE T HE DIRECTION OF3TRAVEL. AS HE TURNED HE EXPERIENCED A SUDDEN PAIN IN HIS LEFT KNEE. VICTIM STATED THAT HE DID NOT FALL OR SLIP. HE SAID HE TWISTED HIS KNEE.
OPERATING THE L9FT SIDE HEAD OF A DUAL HEAD ROOF BOLTING MACHINE.HE PLACED HIS HAND BETWEEN THE ROOF AND A ROOF AND A ROOF BOLT,AND HALF-HEADER.HE INADVERTENTLY ACTIVATED THE BOOM RAISE LEVER AS HE WAS REACH3NG FOR A ROOF BOLT WRENCH.THIS RESULTED IN THE VICTIM'S LEFT HAND BEING CAUGHT BETWEEN THE ROOF AND THE ROOF BOLT ASSEMBLY.
VICTIM WAS ATTE4PTING TO TAKE A TRAILING CABLE AND WATER LINE DOWN FROM THE ROOF WHERE THEY HAD BEEN SUSPENDED ON CABLE HANGERS.THE CABLE AND LINE FELL AND RESULTED IN THE VICTIM EXPERIENCING PAIN IN HIS RIG2T SHOULDER,RIGHT ARM AND THE RIGHT PORTION OF HIS NECK.VICTIM MISSED THE FIRST SHIFT AS A RESULT OF THIS ACCIDENT ON 7-13-87.
VICTIM WAS REHA3GING TRAILING CABLE AND WATERLINE OF JEFFREY 102HC C/M ON CABLE HANGERS WITH THE ASSISTANCE OF 2 CO-WORKERS. THIS TASK WAS BEING PERFORMED TO FACILITATE HAULAGE. FANNIN STATED THAT HE EXPERIE3CED A SUDDEN PAIN IN HIS LOWER BACK WHILE LIFTING ON THE C ABLE AND WATER LINE

HE WAS MOVING THE TRAILING CABLE AND WATER LINE FROM THE ROADWAY TO A POSITION NEAR THE RIGH T RIB TO FACILI7ATE HAULAGE. HE FELT A SUDDEN PAIN IN THE LEFT GROIN AREA, WHILE PULLING ONTHE TRAILING CABLE AND WATER LINE
VICTIM WAS PULL3NG THE TRAILING CABLE OFF THE REEL OF A JOY 21SC SHUTTLE CAR.HE HAD THE CABLE OVER HIS RIGHT SHOULDER AND WAS PULLING IN A FORWARD DIRECTION AWAY FROM THE SHUTTLE CAR.THE CABL E MOMENTARILY F2ULED ON THE CABLE REEL AND THE VICTIM EXPERIENCED A SUDDEN PAIN IN THE THORACIC REGION OF HIS BACK.
VICTIM WAS RIDI3G IN THE CARGO SPACE OF A DIESEL TELETRAM THAT WAS BEING USED AS A MANTRIP. THE MANTRIP WAS ENROUTE FROM THE SURFACE TO THE FACE. THE VICTIM REPORTED THAT HE WAS JOLTED IN HIS SEAT WHEN THE 4ANTRIP HAD TRAVELED APPROX. 70' INBY THE PORTAL. VICTIM STATED THAT HE EXPERIENCED A SUDDEN PAIN IN HIS BACK. VITIM BELIEVES THAT THE MANTRIP RAN OVER AN OBJECT IN THE ROADWAY.

OPERATING CM. H9AD ENCOUNTERED SANDSTONE WHILE HE WAS CUTTING BOTTOM.RIPPER HEAD HUNG MOMENTARILY IN BOTTOM.THIS RESULTED IN MACHINE MOVING SUDDENLY AND UNEXPECTEDLY.HE WAS JOLTED IN OPERATORS COMPARTMENT. 3
VICTIM WAS OPER5TING A 21SC SHUTTLE CAR TRANSPORTING COAL FROM THE FACE TO THE BELT FEEDER. THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY AND RESULTED IN THE VICTIM BEING JOLED IN HIS SEA T. HE REPORTED 1AIN IN HIS LEFT ARM, LEFT LEG, AND LOWER BACK.
VICTIM WAS STAN8ING NEAR A 1 1/4 DISCHARGE LINE FOR A SUMP PUMP. HE WAS ASSEMBLING ROOF BOLTS AND PLATES AT THE TIME. A SHUTTLE CAR MOVING TOWARD THE BELT FEEDER CAUGHT THE DISCHARGE LINE AND CAUSED IT TO S7RIKE THE VICTIM'S FEET. HE LOST HIS BALANCE AND FELL AGAINST THE ROOF BOLTING MACHINE. VICTIM COMPLAINED OF PAIN IN HIS BACK AND LEFT ELBOW.
