Employee was getting in his vehicle at the end of his shift, while putting his foot on the running board, his foot slipped causing him to twist his other ankle. He went home and called his foreman to report the incident.

Employee was attempting to clean his drill steel, when he struck it against the pot it bounce back striking employee on the nose causing a 1/4 to 1/2 inch laceration that required 2 to 3 stitches. Employee was transported to HRMC ER at Prestonsburg KY, and was released later that evening.

Member was working on a tram box, and was kneeling on the ground and a piece of rock fell from the rib striking his hard hat and pushing it onto his nose, Pt was treated on scene by MET. and transported self to HRMC ER at Prestonsburg KY,where he received stitches to a laceration on his nose and diagnosed with a fracture to his nose.

At approximately 12:30 am several large pieces of the high wall dislodged due to the recent freezing and thawing. The rock struck the top of the belt entry canopy,crushing it and damaging the belt structure under it. A plan to remove the damaged canopy and belt structure was put in place and approved by MSHA and the OMSL. There were no injuries in conjunction with this incident.

A non-injury roof fall occurred in the old works of the Northeast Mains Panel 36. The fall was discovered during an examination of the areas. 90'in length,18'in width, and 5' to 6' in height. The fall was investigated by MSHA officials.

EE is 72 inches tall, the seam height is about 60 inches, and the floor is relatively dry. EE was attempting to move the miner cable and waterline, while moving waterline/miner cable, EE felt a pain in his lower back and heard a pop in his back.

Employee shovel belt front of tailpiece 1 section need to cross belt. Turn belt off to cross, it started. Caught on back-stop at transfer point, injure rt leg. Belt examiner turn belt off, switch in ON position. Employee taken to E.R. x-ray positive. He accidenly turn belt on while crossing. Switch examine no defect. Evidence point to human error.

EE walked behind the continuous miner to check his centerline. While walking back to his position to load another sc, he tripped over the miners waterline and fell on his left knee. He finished his shift, and worked the remainder of the week, but his knee continued to bother him. Went to his doctor 10-25-2010 and was diagnosed with a sprained meniscus, taken off work for a week.

Employee walking to left CM. Went through fly pad and left SC coming from #1 to #2, cable slid across #2 heading, struck right foot below ankle. Went to P'burg HRMC ER for treatment, exam inclusive, went to specalist on 06-14-2010, exam revealed fracture to small bone in right foot. Employee told to return 07/06/2010 for follow-up.

THE ROOFBOLTER HAS TRACKS INSTEAD OF WHEELS, AND WAS BACKING OUT OF THE #7 ENTRY ON THE 002 SECTION PREPARING TO MOVE TO THE #8 ENTRY. HE LOST HIS FOOTING AND SLIPPED AND THE ROOFBOLTER CAME DOWN ON HIS LEG.

Employee was filling in for absent RB operator & had bent bolt to install, and was straightening it. Bolt hd. & chuck not aligned. Employee bumped rotation to align bolt & chuck; caused bolt to spin - plate struck right forearm caused injury. Employee taken to E.R. for treatment.

Employee was bringing a two piece drill steel out of the hole when the bottom part of the drill steel came loose and struck employee on left side of face causing a laceration. Employee was later told he had suffered a fracture to his left sinus.
