Voyager #7: 1 recorded fatality Coal

Employee was getting in his vehicle at the end of his shift, while putting his foot on the running board, his foot slipped causing him to twist his other ankle. He went home and called his foreman to report the incident.

A non-injury roof fall occurred at this mine approximately 1 break inby spad # 4161. The airway or travelway was not impeded. Area was breakered off and supplemental support was installed.

A non-injury roof fall occurred in the old # 1 section (001-0 MMU) of this mine, at spad # 246. The fall was approximately 5' to 6'H X 18'W X 40'L. The fall has been breakered off.

Employee was dropping a S/C anchor when he twisted and pulled something in his back. Said he felt a sharp pain in his lower back when he pulled on the cable for slack.

Employee was loading roof bolts and plates onto the tray of the roof bolter when his right hand was lacerated on a roof bolt plate requiring stitches.

Employee was connecting black water line together when the water line slipped out of the coupling striking the employee in the mouth, breaking a tooth.

Employee was attempting to clean his drill steel, when he struck it against the pot it bounce back striking employee on the nose causing a 1/4 to 1/2 inch laceration that required 2 to 3 stitches. Employee was transported to HRMC ER at Prestonsburg KY, and was released later that evening.

Employee was pulling metal bands off of a bundle of roof bolts when band pulled free it struck employee in arm resulting in a laceration.

Member was working on a tram box, and was kneeling on the ground and a piece of rock fell from the rib striking his hard hat and pushing it onto his nose, Pt was treated on scene by MET. and transported self to HRMC ER at Prestonsburg KY,where he received stitches to a laceration on his nose and diagnosed with a fracture to his nose.

A non injury roof fall was discovered by mine examiner while making airways reported it at 12:45 pm 4/27/2011.MSHA was notified by Mine Management.

At approximately 12:30 am several large pieces of the high wall dislodged due to the recent freezing and thawing. The rock struck the top of the belt entry canopy,crushing it and damaging the belt structure under it. A plan to remove the damaged canopy and belt structure was put in place and approved by MSHA and the OMSL. There were no injuries in conjunction with this incident.

A non-injury roof fall occurred in the old works of the Northeast Mains Panel 36. The fall was discovered during an examination of the areas. 90'in length,18'in width, and 5' to 6' in height. The fall was investigated by MSHA officials.

EE stated he was in the process of loading a new scoop door onto a 4 wheeler at the end of the track, while loading the scoop door, EE felt pain in his lower back.

EE is 72 inches tall, the seam height is about 60 inches, and the floor is relatively dry. EE was attempting to move the miner cable and waterline, while moving waterline/miner cable, EE felt a pain in his lower back and heard a pop in his back.

Employee shovel belt front of tailpiece 1 section need to cross belt. Turn belt off to cross, it started. Caught on back-stop at transfer point, injure rt leg. Belt examiner turn belt off, switch in ON position. Employee taken to E.R. x-ray positive. He accidenly turn belt on while crossing. Switch examine no defect. Evidence point to human error.

EE walked behind the continuous miner to check his centerline. While walking back to his position to load another sc, he tripped over the miners waterline and fell on his left knee. He finished his shift, and worked the remainder of the week, but his knee continued to bother him. Went to his doctor 10-25-2010 and was diagnosed with a sprained meniscus, taken off work for a week.

Member was removing top structure and dropped structure on left middle finger. Went to hospital after work and recieved 10 stitches.

Miner was using a jack to straighten rail, jack slipped ,struck miner on rt.side of head,transported miner to a waiting ambulance for transport to area hospital where he recieved stitches to lacerations.

Employee walking to left CM. Went through fly pad and left SC coming from #1 to #2, cable slid across #2 heading, struck right foot below ankle. Went to P'burg HRMC ER for treatment, exam inclusive, went to specalist on 06-14-2010, exam revealed fracture to small bone in right foot. Employee told to return 07/06/2010 for follow-up.

EE was cutting a cardboard box to make it a smaller size to fit into a garbage can, the utility knife slipped and sliced into his hand, first aid was administered at mine site,EE drove himself to Highlands Regional Hospital at Prestonsburg Ky, where he recieved stitches to his hand.

PIECE OF DRAW ROCK FELL, HIT CANOPY BUSTED AND PIECE OF ROCK HIT RIGHT SIDE OF FACE. WAS TAKEN TO ER WAS EXAMINED AND PERSCRIBED ANTIBIOTIC OINTMENT, SCHEDULED TO SEE SPECIALIST ON 04/01/2010 AND WAS RELEASED TO GO BACK TO WORK 04/02/2010.

THE ROOFBOLTER HAS TRACKS INSTEAD OF WHEELS, AND WAS BACKING OUT OF THE #7 ENTRY ON THE 002 SECTION PREPARING TO MOVE TO THE #8 ENTRY. HE LOST HIS FOOTING AND SLIPPED AND THE ROOFBOLTER CAME DOWN ON HIS LEG.

Employee was watering scoop batteries, the lid fell and struck employee in mouth, cutting lip and chipped front teeth.

Rock measuring 5 1/2' X 10' X 18' wide fell ont CM while cutting coal. No vent. or travel of people or equip were affected. Both MSHA & OMSL were notified, and investigated.

Employee was filling in for absent RB operator & had bent bolt to install, and was straightening it. Bolt hd. & chuck not aligned. Employee bumped rotation to align bolt & chuck; caused bolt to spin - plate struck right forearm caused injury. Employee taken to E.R. for treatment.

Employee bent bolt to install, feeding bolt into roof, bolt kick out of drill pot striking left wrist. Continued to work until 4-13-09. Went to Dr. then taken off work.

Employee was bringing a two piece drill steel out of the hole when the bottom part of the drill steel came loose and struck employee on left side of face causing a laceration. Employee was later told he had suffered a fracture to his left sinus.
