EE was exiting skid steer, foot caught door frame threw EE off balance when landed on right foot knee popped. Thirty minutes later knee started hurting and swelling. EE told lead person EE did not want medical attention. On 4/12/18 EE decided EE needed to see physician. EE was put on restricted duty. EE had torn ACL in right knee at previous Job.

He was backing up a D-9 Dozer, ran over a rock. He said it pulled his neck.
EE was shoveling at the end of the Finishing Screen. A rock fell, hitting him on the right shoulder. He reported it to his supervisor. He said he was ok and went back to work. On 2/15/07, one hour before work time, he called in saying he was going to the doctor because his shoulder was hurting. The doctor excused him from work for 2 days.

Employee was going to the pit to haul shot rock. We had a lot of rain the day before. The road had been washed out underneath. The front of the truck dropped in the hole making a sudden stop. Employee hit her knees on the dash.

EE WAS SHOVELING TAIL PULLEY. CONVEYOR OVERHEADHAD GONE EMPTY. A ROCK ROLLED OFF SIDE OF CONVEYOR, STRIKING EE'S HARD HAT AND MADE A LACERATION ON RIGHT SIDE OF SCALP. HE HAD TO GET 6 STITC HES. EMPLOYER WELDED SIDE PLATES ON CONVEYOR TOPREVENT ANY OTHER ACCIDENTS.
