Employee was checking the water meter gauge on the hydrosizer when employee's leg slipped through the gap in the deck/stairs to the hydrosizer.

Employee was welding in dead cell (tied off appropriately) and welding machine was grounded appropriately. The wire to the machine was tugged on at the bottom, and the wire became compromised and shocked EE.

The laborer was feeding a conveyor belt through rollers. The conveyor belt was lowered completely to the ground while the miner was working on it. The miner pulled too hard on the conveyor at the same time that another miner was pulling on it and their hand went between the rollers and the conveyor belt.
