ee had just finished breaking a slab into blocks. ee was putting a Retractable back in the storage box it hit the top of the box causing it to come down and punch their finger.

EE was getting out of the forklift, they extended their arms up when they felt a pop in their lower back. Supervisor was not on site and the communication misunderstood in regard to the injury.

employee was pushing 7 pieces of curb from the splitter to the plunge saw, with another person, when EE felt a pop, EE stopped instantly.

EE was sliding a small and thin piece of grout (around 10 lbs) on the conveyor in order to grab it. While EE was doing so, it flipped while falling between the two sections of conveyor and the sharp edge cut EE's wrist (above glove line).

Employee was pulling a piece of curb (with an end piece attached, coming out of the straight joint saw). While they were pulling, the end piece broke and fell right on the top of their right foot. The end piece was weighing around 90 lbs.

Employee was handling curb with it tipped over and EE's finger was caught between. EE was wearing gloves, while pulling hand out EE's finger got scratched. Friction of the glove and pressure under stone is what caused the scratch on finger. EE wanted to go to the doctor. Doctor administered tetanus shot and antibiotics. First aid was administered at site.

On Friday 9/6/2024, EE was working on the joint saw, which involves going up and down a couple of steps quite often. By mid-day, EE reported to co-worker that their knee was starting to hurt because of the steps. Co-worker moved EE to another position. On 9/11/2024, EE told another co-worker that knee pain was still there and asked to go get it checked out at the clinic.

EE bent over to fit the chain underneath a piece of granite that was on the conveyor and felt a sharp pain in EE's lower back.

EE was trimming a piece of radius curbing and EE missed the chisel and hit the top of EE's left hand with the hammer and forced EE's hand on the stone, the knuckles took the hit.

Employee was clearing grout and received a puncture wound on left 3rd digit finger.

Injury was declared on 8/6/24 @ 7:15 AM. EE was cleaning the radius splitter pit & while dragging some small granite shavings out of the trays, a splinter made its way through the glove into middle finger. EE didn't think it was anything serious & didn't declare it right away, but realized a few days later that it might be infected & asked to go get it checked out.

Miner was attempting to move a bunk which the granite block sits on during the cutting process with a "hook tool" when the hook broke causing the miner to fall backwards.

Miner does not know how or what happened to receive the injury

Miner was assisting with placing a lift chain loop onto a overhead crane hook when the chain loop pinched EE's right thumb between the loop and hook causing an avulsion to the right thumb pad.

Miner was lifting grout felt a pain in groin area.

Caught in, under, between object handled. *Miner was moving grout into a grout box. When EE went to drop the grout, EE's glove got caught on an edge and it pulled EE's hand down onto another piece of grout.

Miner tried to push a piece of grout laying under the splitter to an area where the crane could pick it up and EE strained lower back.

Miner reported that after a medical procedure they feels their condition is posable silicosis based on a pervious diagnosis in 2015.

EE stepped on a plank that was acting as the floor of the splitter pit and the plank broke and EE fell and bruised EE's left calf

While attempting to move a piece of curb of a conveyor with a 3/8" grout chain the hook caught on the conveyor as the piece was being hoisted and the chain became tight and started to lift the conveyor. The operator stopped and placed a hand on the chain as it was being lowered and while the chain was still under tension, it dislodged from the conveyor and hit forearm

EE was working on curbing on the end of the conveyor line. EE grabbed a piece of grout to throw it into the bin, but the piece was in contact with EE's forearm and cut EE when EE released it.

Lifting end cuts from the radius line and turned to throw a piece into a grout box and injured EE's back

Cleaning the floor of the saw shed with the bobcat (small tractor). EE wanted to put a piece of grout - by hand - in the bucket of the bobcat and hurt back by trying to lift/drag this piece.

While doing secondary breakage operations EE struck a wedge and EE struck off and a piece sheared off lodging in EE's left forearm

While walking by a stone being split the individual swinging the sledge hammer struck off and debris hit the injured person in the chin area causing a laceration injury

Setting up a Marini drill and EE's back started to hurt towards the end of the shift. Possible back strain. Miner had drilled anchor point holes with a plug drill.

Using a bullset on a piece of curb and when the set was hit with the hammer a piece of either steel or stone struck EE's face causing and imbedded foreign object

EE ONSITE TO DUMP EXCAVATION MATERIAL. DUMPED LOAD THEN HAD TRACTION PROBS WHEN MOVING OUT. GOT OUT OF TRUCK TO SHOVEL SAND UNDER REAR TIRES THEN PUT SHOVEL BACK. WHEN TRUCK HADN'T MOVED YET, SWENSON FORKLIFT OPER. CHECKED ON HIM AND FOUND HIM OUTSIDE HIS TRUCK, COLLAPSED ON GROUND & UNRESPONSIVE. THAT OPER.IMMEDIATELY BEGAN CPR AND AN AMBULANCE WAS DISPATCHED-ARRIVED AT 1:18PM.

Employee was cutting curb using a steel hand chisel. A piece of steel broke off and went into his left knee.

Employee was driving a truck or a loader all day - steering all day.

Employee was reaching for something and his hand was caught in part of the machinery.

Employee was drilling granite. The bit broke and the drill moved and the remaining piece of drillbit went into his foot, just behind the steel toe cap.

A block was being lifted from the quarry. The chain tightened and a small piece of stone (corner) broke off and flew over and hit his arm.

Employee was lifting a piece of curbing from the curb line and felt a "twinge". (Employee worked full time through 7/11/08 and was operated on 7/14/08).

Employee was pushing curb on the roller/conveyor line, when one piece tipped over and fell on his finger.

Employee was leaning his hip against a piece of curb he was cutting, when it broke and he was pushed backwards and felt pain in his lower back/right leg.

Employee was holding the bullset when the granite broke and he moved his hand back, alongside the top edge of the granite curb. REQUIRED STITCHES.

Employee was cutting curb using a hand set. A piece of metal from the hand set flew off when he hit the granite. It lodged in his stomach area.

Employee was holding a hand "bull set" and another employee was hitting it w/a sledge hammer. A piece of granite broke off the curbing and a chip hit the top of his right hand.

Between 8/13/07 and 8/20/07, employee was doing a task (burning slabs) where he had to be a little "bent over" all day. Back symptoms got progressively worse. (NOTE: worked until 8/31/07 - lost time started 9/4/07).

Employee was using two wrenches - pulling each one - and felt something "let go" in his left chest/shoulder area.

Employee was walking in the Curb Shed towards his saw when he stepped on a piece of granite on the floor.

Employee was standing on drilling platform, moving his drill - his foot slipped and he "caught" himself with his right hand.

Employee was unchaining a block when his hand slipped off the chain and hit the edge of the granite piece.

Employee was using a long steel bar to clear the channel in the quarry - where the burner was having a hard time burning away the granite.

Employee was chaining a quarry block, the chains were tightened when lifting and his fingers were caught between the chain and the block.

Employee says first symptoms began early Spring (thus 4/1/06 date). Repetitive use of jackhammer resulting in carpal tunnel symptoms. Reported the discomfort to supervisor on 8/21/06 - when it had escalated to a point where he could not work.

Employee was positioning the Nova Drill - tried to push it to pivot the last 2 legs into place when he slipped and twisted his back.

Employee was climbing the ladder into the back of the Euclid dump truck to unhook a chain on a grout box, slipped and his shoulder hit the grout box.

Employee had unhooked the chain from the burner cabinet after moving it and was closing the door on the cabinet when the chain swung out and hit his hardhat.

Employee was picking up a small piece of granite when his hand slipped and landed underneath the stone on the quarry floor.

Employee was putting a wooden timber on a piece of curb, in order to put more on top of the timber, when he lost his hold on the timber and it came down on his thumb.

Employee was pulling a piece of granite curb on the rollers when it fell onto its side. Employee jumped back and "wrenched" his hand (strained left wrist). (NOTE: Employee returned to work next day so I did not file MSHA. Noticed my oversight now, when doing my year-end summaries).

EE was repositioning 12' steel drill rod in quarry channel. Felt tightening in his shoulder.

Employee was burning curb and felt his back getting tighter the longer he worked. Felt a tightness in his back.

Employee was burning the curbing and came to the end of the row. He "threw" the hose over the end and felt a pull in his back.

EMPLOYEE WAS CHAINING A PIECE OF GRANITE, THE CHAIN SLIPPED AND HE TRIED TO REPOSITION IT AND FELT SOMETHING IN HIS LEFT SHOULDER.

EE WAS USING THE BURNER ON A PIECE OF CURBING. THE REGULATOR FOR THE FUEL OIL CAME LOOSE, EE ATTEMPTED TO FIT IT, AND THE TORCH SLIPPED ACROSS HIS HAND.

EE WAS MOVING THE METAL COVER ONTO THE SPLITTER WHEN HE FELT A SHARP PAIN IN HIS BACK.

EMPLOYEE WAS SHOVELING, FELL ON THE ICE. FELL ONTO HIS BACK.

EE WAS MOVING EQUIPMENT IN THE QUARRY. THE STEEL WAND FROM THE PROPANE HEATER FELL FROM THE LEDGE WHERE HE HAD STORED IT AND HIT HIM ABOVE HIS MOUTH.

EE WAS PULLING A DRILL ROD FROM THE GRANITE WHEN HIS HAND SLIPPED AND HIS ARM WENT UP AGAINST THE LEG OF THE DRILLING MACHINE.

EE WAS CHAINING A GRANITE BLOCK INT HE QUARRY WHEN THE CHAIN SLIPPED AND HIS ARM DRAGGED ALONG THE STONE.

EE WAS LIFTING A RAILING TO REPOSITION IT INTO ANOTHER HOLE WHEN HE FELT SOMETHING IN HIS SHOULDER.

EE WAS PICKING UP A PIECE OF GRANITE AND FELT PRESURE.

EE WAS JUMPING DOWN FROM THE TRACKS ON THE CRANE. FELT SOMETHING WRONG IN IS KNEE WHEN HE LANDED ON THE GROUND.

EMPLOYEE WAS TRYING TO FREE A STUCK METAL WEDGE FROM THE GRANITE USING A SLEDGEHAMMER. HIS FINGER GOT IN THE WAY.

EMPLOYEE WAS PICKING UP SMALL PIECES OF GRANITE TO GET THEM OUT OF THE WAY WHEN ONE SLIPPED DOWN THE FRONT OF HIS HAND.

EMPLOYEE WAS SETTING UP HIS NOVA DRILL WITH THE BAR. THE BAR SLIPPED AND HE FELL AGAINST THE LEG OF THE DRILL.

EE HAD BEEN USING A HAMMER TO FINISH THE CURBING AND WAS PUSHING THE CURBING ON ROLLERS. HE STARTED TO HAVE PAIN IN JUNE 2002, AND TRIED TO WORK AND THOUGHT IT WOULD GET BETTER. ON 7/22/02 HE MADE AN APPOINTMENT W/HIS DR. AS IT WAS WORSE.

EMPLOYEE WAS PULLING A WOODEN TIMBER FROM THE TOP OF THE PILE OF CURB AND THE PIECE OF CURB MOVED A BIT AND SLID OFF ONTO HIS FOOT.

EE WAS CHANGING THE POSITION OF THE DRILL. HAD PUT THE AIR LINE DOWN OVER THE RAILING. HE HIT THE BY PASS VALVE ON THE HOSE AND THEN THE HOSE HIT HIM ON THE FOREHEAD.

WHILE MOVING THE CRANE TO ANOTHER LOCATION, THERE WAS A MECHANICAL FAILURE. EE ELECTED TO GET OUT OF THE CRANE WHILE STILL MOVING.

RUNNING A BURNER ON A GRANITE BENCH AND THE BENCH SLID DOWN 20' TAKING EQUIPMENT & EE.

EMPLOYEE WAS DRILLING, PUSHING AGAINST THE DRILL WITH HIS LEG (REPETITIVE WORK).

EE WAS SHOVELING GRANITE FROM UNDERNEATH SPLITTER WHEN HE STARTED HAVING UPPER RIGHT SHOULDER & BACK PAIN.

EE WAS PUSHING CURB ON THE CONVEYOR ROLLERS. TO STOP THE PIECE OF CURB HE PUT A SMALL PIECE OF GRANITE INTO THE ROLLER AND THE CURB THEN HIT HIS FINGER.

EE WAS MOVING A PIECE OF CURBING WHEN IT TIPPED AND FELL ON HIS FINGER.

EE WAS RUNNING PLATFORM DRILL & USING SLEDGE HAMMER-REPETITIVE MOTION ACTIVITIES.

EE WAS TURNING OVER A PIECE OF CURB ON THE BANKERS WHEN IT TIPPED AND HE WENT TO GRAB IT AND IT CUT HIS HAND WHEN IT SLID DOWN.

EE WAS CLIMBING THE LADDERS OUT OF THE QUARRY WHEN HE FELT A TIGHTENING IN THE BACK OF HIS RIGHT KNEE.

EE WAS JUMPING OUT OF A PICK-UP TRUCK BED WHEN HIS FOOT CAUGHT AND HE LANDED ON THE GROUND ON ONE FOOT AND THEN TWISTED HIS BODY.

EE WAS PUSHING THE GRANITE CURBING PIECES ON THE ROLLERS.

EE WAS WORKING ON THE CURB LINE AROUND 5-1-01 (PUSHI8NG 7 PULLING GRANITE ON ROLLERS AND STANDING PIECES UPRIGHT). NOTICED A PAIN IN HIS STOMACH-AND IT HAS GOTTEN MUCH WORSE LATELY.

EE WAS PICKING UP A PIECE OF GRANITE TO PUT INTOTHE GROUT PILE AND FELT SOMETHING FUNNY IN HIS BACK.

EE WAS WALKING IN FRONT OF THE DIAMOND SAW BUILDING WHEN HE SLIPPED ON THE SNOW AND FELL.

EE WAS TURNING PIECES OF CURB ON THE BANKERS WHEN ONE FELL OVER TOO CLOSE TO THE NEXT PIECE AND HIS FINGER WAS SCRAPED.

EE WAS CHAINING A BLOCK WHEN THE WIND TOOK THE CHAIN HOOK AND HIT HIS HAND UP AGAINST THE BLOCK.

EMPLOYEE WAS MOVING A PIECE OF CURB ON THE BANKERS AND FELT A PAIN IN HIS LOWER BACK.

EE WAS HITTING A WEDGE WITH A SLEDGEHAMMER AND THE HAMMER MISSED THE WEDGE & HIT HIS THUMB

EE WAS KNOCKING BITS OFF THE DRILL STEEL WITH A HAMMER WHEN A PIECE OF STEEL BROKE OFF THE HAMMER AND HIT HIS LEG.

SHOVELING GROUT ON 03/01/00 AND RUNNING BLOW PIPE. FELT PAIN GOING DOWN RIGHT ARM, LEG AND LOWER BACK.

EMPLOYEE HAD UNHITCHED CHAIN FROM BOX, BEND DOWNTO PICK UP THE RADIO HE DROPPED, THE WIND CAUGHTAND WHEN HE STOOD UP THE HOOK HIT HIM ON HIS HARDHAT.

EMPLOYEE WAS WALKING IN THE QUARRY YARD TO THE SUPPLY TRAILER. ANOTHER EMPLOYEE WAS BACKING UP IN HIS PICK UP TRUCK, DIDN'T SEE HIM WALKING, ANDBACKED INTO HIM.

EE WAS USING A BAR TO REPOSITION THE LIFT DRILL WHEN THE BAR SLIPPED AND HIS HAND HIT UP AGAINST THE DRILL.

EMPLOYEE WAS TOWING A TRUCK WITH A LOADER. EMPLOYEE HAD JUST UNHOOKED THE TRUCK, AND WAS WALKING TOWARD THE LOADER TO UNHOOK IT, WHEN THE TRUCK ROLLED TOWARD HIM, PINNING HIM BETWEEN THE TWO V EHICLES.

EMPLOYEE WAS PULLING HOSES FOR THE DUST COLLECTOR. HE LOST HIS BALANCE AND FELL TO THE GROUND.

EMPLOYEE WAS WALKING IN THE QUARRY, SLIPPED AND FELL ON ICE.

NOTE: EE HAS NOTICED PAIN IN BOTH WRISTS & ARMS FOR ABOUT 2 YEARS, BUT VERY PAINFUL LATELY. THERE HAS BEEN NO LOST TIME AT ALL FOR THIS CONDITION, BUT HE HAS BEEN REFERRED TO A SPECIALIST. EE WAS SHOVELING ABRASIVE IN 1994 AND NOW IS SHOVELING PLASTER FOR THE DIAMOND SAWS (SETTING UP THE BLOCK TO BE SAWN).

EE WAS USING WEDGES AND A SLEDGEHAMMER TO CUT OFF A SMALL PIECE OF GRANITE WHEN THE PIECE BROKE OFF, BOUNCED ON THE GROUND AND THEN HIS HIS FOOT.

EE WAS PULLING ON A GRANITE SLAB WHEN HE HURT HIS BACK.

EE WAS MOVING WOODEN BLOCKING IN THE QUARRY. HE SIGNALLED FOR THE PIECE OF GRANITE TO BE LOWERE AND HIS FINGER GOT "PINCHED" BETWEEN THE WOOD AND THE GRANITE PIECE.

EMPLOYEE WAS WALKING IN THE QUARRY, TRIPPED AND FELL AND A WOODEN BOARD JABBED HIS ARM.

EE WAS HOLDING A STEEL ROD ONTO THE DRILL RIG AS THEY WERE TRYING TO REMOVE A BOLT. ANOTHER EE WAS HITTING THE ROD WITH A SLEDGE HAMMER AND MISSED.

EMPLOYEE WAS LIFTING A ROCK, BUILDING A RETAINING WALL.

EE WAS HITTING A STEEL WEDGE WITH A SLEDGEHAMMER WHEN IT "POPPED" BACK INTO HIS FACE.
