EE was using a log chain to adjust the stone under the radius. EE put the lug too tight against the stone and when EE released the stone to advance it fell back on EE's ankle.
ee had just finished breaking a slab into blocks. ee was putting a Retractable back in the storage box it hit the top of the box causing it to come down and punch their finger.

EE was sliding a small and thin piece of grout (around 10 lbs) on the conveyor in order to grab it. While EE was doing so, it flipped while falling between the two sections of conveyor and the sharp edge cut EE's wrist (above glove line).

Employee was pulling a piece of curb (with an end piece attached, coming out of the straight joint saw). While they were pulling, the end piece broke and fell right on the top of their right foot. The end piece was weighing around 90 lbs.

Employee was handling curb with it tipped over and EE's finger was caught between. EE was wearing gloves, while pulling hand out EE's finger got scratched. Friction of the glove and pressure under stone is what caused the scratch on finger. EE wanted to go to the doctor. Doctor administered tetanus shot and antibiotics. First aid was administered at site.

On Friday 9/6/2024, EE was working on the joint saw, which involves going up and down a couple of steps quite often. By mid-day, EE reported to co-worker that their knee was starting to hurt because of the steps. Co-worker moved EE to another position. On 9/11/2024, EE told another co-worker that knee pain was still there and asked to go get it checked out at the clinic.

Injury was declared on 8/6/24 @ 7:15 AM. EE was cleaning the radius splitter pit & while dragging some small granite shavings out of the trays, a splinter made its way through the glove into middle finger. EE didn't think it was anything serious & didn't declare it right away, but realized a few days later that it might be infected & asked to go get it checked out.

Miner was attempting to move a bunk which the granite block sits on during the cutting process with a "hook tool" when the hook broke causing the miner to fall backwards.
Miner was assisting with placing a lift chain loop onto a overhead crane hook when the chain loop pinched EE's right thumb between the loop and hook causing an avulsion to the right thumb pad.
Caught in, under, between object handled. *Miner was moving grout into a grout box. When EE went to drop the grout, EE's glove got caught on an edge and it pulled EE's hand down onto another piece of grout.

While attempting to move a piece of curb of a conveyor with a 3/8" grout chain the hook caught on the conveyor as the piece was being hoisted and the chain became tight and started to lift the conveyor. The operator stopped and placed a hand on the chain as it was being lowered and while the chain was still under tension, it dislodged from the conveyor and hit forearm

EE was working on curbing on the end of the conveyor line. EE grabbed a piece of grout to throw it into the bin, but the piece was in contact with EE's forearm and cut EE when EE released it.
Cleaning the floor of the saw shed with the bobcat (small tractor). EE wanted to put a piece of grout - by hand - in the bucket of the bobcat and hurt back by trying to lift/drag this piece.
While walking by a stone being split the individual swinging the sledge hammer struck off and debris hit the injured person in the chin area causing a laceration injury

Piece of steel chipped off hammer employee was using on granite slabs, hit in chest (left side). X-rays not clear at first if foreign object in body, reread and recommending removal of foreign object.
EE ONSITE TO DUMP EXCAVATION MATERIAL. DUMPED LOAD THEN HAD TRACTION PROBS WHEN MOVING OUT. GOT OUT OF TRUCK TO SHOVEL SAND UNDER REAR TIRES THEN PUT SHOVEL BACK. WHEN TRUCK HADN'T MOVED YET, SWENSON FORKLIFT OPER. CHECKED ON HIM AND FOUND HIM OUTSIDE HIS TRUCK, COLLAPSED ON GROUND & UNRESPONSIVE. THAT OPER.IMMEDIATELY BEGAN CPR AND AN AMBULANCE WAS DISPATCHED-ARRIVED AT 1:18PM.

Employee had unhooked the chain from the burner cabinet after moving it and was closing the door on the cabinet when the chain swung out and hit his hardhat.

EMPLOYEE WAS WALKING IN THE QUARRY YARD TO THE SUPPLY TRAILER. ANOTHER EMPLOYEE WAS BACKING UP IN HIS PICK UP TRUCK, DIDN'T SEE HIM WALKING, ANDBACKED INTO HIM.
EMPLOYEE WAS TOWING A TRUCK WITH A LOADER. EMPLOYEE HAD JUST UNHOOKED THE TRUCK, AND WAS WALKING TOWARD THE LOADER TO UNHOOK IT, WHEN THE TRUCK ROLLED TOWARD HIM, PINNING HIM BETWEEN THE TWO V EHICLES.
NOTE: EE HAS NOTICED PAIN IN BOTH WRISTS & ARMS FOR ABOUT 2 YEARS, BUT VERY PAINFUL LATELY. THERE HAS BEEN NO LOST TIME AT ALL FOR THIS CONDITION, BUT HE HAS BEEN REFERRED TO A SPECIALIST. EE WAS SHOVELING ABRASIVE IN 1994 AND NOW IS SHOVELING PLASTER FOR THE DIAMOND SAWS (SETTING UP THE BLOCK TO BE SAWN).
EE WAS MOVING WOODEN BLOCKING IN THE QUARRY. HE SIGNALLED FOR THE PIECE OF GRANITE TO BE LOWERE AND HIS FINGER GOT "PINCHED" BETWEEN THE WOOD AND THE GRANITE PIECE.
EE WAS HITTING A STEEL WEDGE WITH A SLEDGEHAMMER WHEN IT "POPPED" BACK INTO HIS FACE.
