Employee was on a truck ladder adjusting tarp arm. When EE climbed down from the ladder stepping on to the ground EE hurt back.

Employee was moving stone from pallets to the cutting machine rolls when a stone he was moving slipped and pinched his hand between two pieces of stone.

Employee was taking stone from a pallet and placing it on the rolls of the cutting machine, when a piece of stone slipped from the pallet and the employees hand was caught between the rolls and the stone, smashing his middle finger on his left hand.

Employee was palletizing stone from the conveyor belt when a piece of stone started to fall off of the belt. The employee atttempted to catch the stone so it would not hit his leg, in doing this he strained his back.

Struck against stationary object

Booming load down on truck for delivery hit elbow on bed of truck. Elbow sore and appears to be swollen.

Employee was walking in quarry on slight incline and slipped and fell on loose material. Landed on elbow.

Struck by flying object

Employee was working approx. 20 feet from where fork truck was removing stone when a piece of stone approx. 8"" in diameter flew from the fork truck and struck employee in the right hip. No first aid was required. Bruising occured and a minimal amount of swelling and tenderness.

The employee was unloading stone from a pallet for cutting, when a large stone slid from the pallet crushing the employees finger between the stone and the cement floor. Traumatic finger injury to middle finger of right hand.
