EMPLOYEE WAS WALKING DOWN EMBANKMENT W/SMALL WATER PUMP TO BE PALCED IN OPERATION. EMPLOYEE LOST HIS FOOTING AND SLIPPED. FELL ON HIS REAR END AND LOWER BACK CAUSING STRAIN TO THAT AREA.

PLANT SUPERVISO9 DID NOT LOCK OUT AND DEENERGIZE CIRCUIT FEEDING CUTLER HAMMER MOTOR CONTROL BOX PRIOR TO OPENING THE COVER.HE WAS HOLDING A STEEL GUARDED EXTENSION LIGHT IN RIGHT HAND.HE MADE CONTACT WITH O3E PHASE OF 440 VOLT CIRCUIT IN THE BOX.THIS RESULTED IN 2ND DEGREE BURNS TO FACE AND HANDS.

EMPLOYEE WAS WO9KING ON LOADER MOTOR WHILE STANDING ON THE LOADER TIRE, HE THEN LOST HIS BALANCE AND GRABBED THE DOOR OF THE LOADER WHICH WAS NOT SHUT. THE DOOR CAME OPEN CAUSING THE FALL.
